Glimepiride
Indications/Uses
NIDDM (type 2 DM) inadequately controlled by diet, exercise & wt reduction program.
Dosage/Direction for Use
Initial dose & dose titration: 1 mg once daily. May be increased at intervals of 1-2 wk, carried out stepwise as follows: 1 mg-2 mg-3 mg-4 mg-6 mg, & in exceptional cases, 8 mg.
Administration
Should be taken with food: Take immediately before the 1st main meal of the day. Do not skip meals.
Contraindications
Hypersensitivity to glimepiride, other sulfonylurea, sulfonamide, have a risk of hypersensitivity reactions. IDDM, diabetic ketoacidosis or a history of diabetic ketoacidosis, diabetic coma or precoma. Liver & kidney dysfunction. Pregnancy & lactation.
Special Precautions
Increased risk of hypoglycemia in case of unwillingness, irregular meal times, undernutrition fasting, change of dietary pattern, alcohol consumption. Overdose, endocrine disorders affecting carbohydrate metabolims eg, thyrotoxicosis. Impaired kidney function. Severe liver dysfunction.
Adverse Reactions
Hypoglycemia, visual disturbances, nausea, vomiting, feeling of fullness in the epigastrium, abdominal pain, diarrhea, liver dysfunction, hemolytic anemia, erythrocytopenia, leucopenia, granulocytopenia, agranulocytosis, thrombocytopenia, pancytopenia, allergic reactions eg, urticaria or skin redness which may continue or accompanied w/ apnea & hypotension up to shock.
Drug Interactions
Insulin, other oral antidiabetic drugs, ACE inhibitor, allopurinol, anabolic steroid, sex hormones, chloramphenicol, coumarin derivative, MAOIs, miconazole, paracetamol, pentoxifylline (high parenteral dose), phenylbutazone, azopropazone, oxyphenbutazone, probenecid, quinolone, salicylate, sulfipyrazone, sulphonamide, tetracycline, tritoqualine, trofosfamide, acetazolamide, barbiturate, corticosteroid, diazoxide, diuretics, epinephrine (adrenaline), other sympathomimetic drugs, glucagon laxative, nicotinic acid (high dose), estrogen, progesterone, phenothiazine, rifampicin, thyroid hormone, H2 receptor antagonist, clonidine, reserpine, β receptor antagonist, clonidine, guanethidine, coumarin derivative, alcohol.
