Fluphenazine decanoate
Indications/Uses
Long-term management of psychotic disorder eg, chronic schizophrenia.
Dosage/Direction for Use
IM/SC Psychotic disorder Adult & severely agitated patient Initially 12.5-25 mg (0.5-1 mL) then adjust in accordance w/ patient response. Maintenance: Not to exceed 100 mg. If >50 mg is necessary, increase next & succeeding doses in 12.5-mg increments. Childn ¼-1/3 adult dose.
Contraindications
Comatose & severely-depressed states, marked cerebral & hepatic damage, jaundice, atherosclerosis, phaeochromocytoma, existing blood dyscrasias, renal & cardiac insufficiency.
Special Precautions
Monitor ECG & K levels in patients w/ risk factors for ventricular arrhythmias eg, cardiac disease, subarachnoid haemorrhage, metabolic abnormalities (hypokalaemia, hypocalcemia or hypomagnesemia), starvation, alcohol abuse & concomitant w/ QT interval-prolonging drugs. Epilepsy, history of convulsive disorders, Parkinson's disease; thyrotoxicosis, hypothyroidism, liver or severe resp disease; personal or family history of narrow-angle glaucoma, prostatic hypertrophy, myasthenia gravis or high ambient temp; surgery; phenothiazine hypersensitivity. May affect ability to drive or operate machinery. Pregnancy & lactation. Elderly especially frail or at risk of hypothermia.
Adverse Reactions
Acute dystonic reactions, parkinsonian-like states, tardive dyskinesia; drowsiness, lethargy, blurred vision, dry mouth, constipation, urinary hesitancy or incontinence, mild hypotension, impaired judgement & mental skills, epileptiform attacks; abnormal skin pigmentation & lens opacities. ECG changes w/ prolonged QT interval & T-wave changes; serious arrhythmias including ventricular tachycardia & fibrillation. Hypothermia; NMS; hyperprolactinaemia, impaired sexual function; oedema.
Drug Interactions
Alcohol, general anaesth, hypnotics, sedatives, strong analgesics; adrenaline & other sympathomimetics; adrenergic blockers; levodopa, anticonvulsants, TCAs; anticoagulants, antidepressants; lithium; antiparkinsonian, other anticholinergics.
