Indications/Uses
Induction of labor in cases of post-term gestation, premature membrane rupture or preeclampsia; caesarean section; post-partum uterine atony & hemorrhage. Management of incomplete, missed or inevitable abortion.
Dosage/Direction for Use
Induction labor Initially 1-4 milliunit/min (2-8 drops/min), may be gradually increased at intervals not ≤20 min. Caesarean section 5 IU slow IV after delivery. Prevention of postpartum uterine hemorrhage 5 IU slow IV or 5-10 IM after placental delivery. Induction or enhancement of labor: Infusion should be continued at an increased rate during 3rd stage labor & for next few hr thereafter. Treatment of postpartum uterine hemorrhage 5 IU slow IV or 5-10 IU IM, followed in severe cases by IV infusion 5-20 IU of Santocyn in 500 mL of a non-hydrating diluent, run at the rate necessary to control atony. In complete, missed or inevitable abortion 5 IU slow IV or 5-10 IU IM, if necessary followed by IV infusion at a rate of 20-40 milliunit/min.
Contraindications
Hypersensitivity. Spontaneous labor, normal delivery; cephalopelvic disproportion; fetal malpresentation; placenta & vasa praevia; placental abruption; cord presentation or prolapse, overdistension or impaired uterine resistance; invasive cervical cancer; uterine hyperactivity; severe toxemia or CV disorders.
Special Precautions
Tumultous labor. Avoid rapid IV administration.
Adverse Reactions
Tetanic contractions or uterine rupture & soft tissue laceration; fetal bradycardia, arrhythmias & asphyxiation; fetal or maternal death; water intoxication associated w/ hyponatremia; pulmonary edema, convulsions, coma; acute hypotension, flushing & reflux tachycardia; nausea, vomiting, cardiac arrhythmias.
Drug Interactions
Prostaglandins; cyclopropane; halothane; sympathomimetic vasoconstrictor agents.
