Clopidogrel
Indications/Uses
Reduction of atherothrombotic events in MI, ischaemic stroke or established peripheral arterial disease. Patient suffering from non-ST segment elevation acute coronary syndrome (unstable angina or non-Q-wave MI) in combination w/ ASA.
Dosage/Direction for Use
Adult & elderly 75 mg daily. Non-ST segment elevation acute coronary syndrome (unstable angina or non Q-wave MI) Loading dose: 300 mg then followed by 75 mg once a day w/ ASA 75-325 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Active pathological bleeding eg, peptic ulcer or intracranial haemorrhage. Severe liver impairment. Lactation.
Special Precautions
Patients at risk of increased bleeding from trauma, surgery or other pathological conditions; concomitant treatment w/ ASA, NSAIDs, heparin, glycoprotein IIb/IIIa inhibitors, thrombolytics; lesions w/ a propensity to bleed paticularly GI & intraocular; acute MI w/ ST-segment elevation, acute ischaemic stroke (<7 days). Patient w/ renal impairment & moderate hepatic disease who may have bleeding diathesis. Pregnancy.
Adverse Reactions
Severe neutropaenia & thrombocytopaenia, aplastic anaemia; headache, dizziness, paraesthesia, vertigo; GI disturbances; increased bleeding time, decreased platelets; rash, pruritus; leucopenia, decreased neutrophils & eosinophilia; bleeding, GI, eye or intracranial bleeding purpura/bruising/haematoma & epistaxis, haematuria, thrombotic thrombocytopenic purpura; anaphylactoid reactions; confusion, hallucinations; taste disturbance; vasculitis, hypotension; bronchospasm; colitis, pancreatitis; hepatitis; angioedema, bullous dermatitis, erythematous rash, urticaria, eczema, lichen planus; arthralgia, arthritis; glomerulonephritis; fever; abnormal liver function test, increased blood creatinine.
Drug Interactions
Increased risk of bleeding w/ warfarin, glycoprotein IIb/IIIa inhibitors, ASA, heparin, thrombolytics & NSAIDs.
