Ceftriaxone
Indications/Uses
Lower resp tract infection & pneumonia; skin, skin structure & soft tissue, bone & joint & intra-abdominal infections; UTI, meningitis, uncomplicated gonorrhea. Prophylaxis of pre-op infections; gonococcal infections; pelvic infections in women; otitis media.
Dosage/Direction for Use
Adult & childn >12 yr Gonorrhea 250 mg IM as a single dose. Peri-op infection prophylaxis 1 g IV 0.5-2 hr pre-op. Other infections 1-2 g IM/IV every 24 hr or 500 mg-1 g every 12 hr. Max: 4 g daily. Childn <12 yr Meningitis Initially 100 mg/kg IM/IV daily followed by 100 mg/kg daily or 50 mg/kg 12 hrly for 7-14 days. Max: 4 g daily. Otitis media 50 mg/kg IM as a single dose. Max: 1 g. Skin & soft tissue infections 50-75 mg/kg 12 hrly or 25-37.5 mg/kg 12 hrly. Max: 2 g daily. Other serious infections 25-37.5 mg/kg IM/IV 12 hrly. Max: 2 g daily.
Contraindications
Hypersensitivity to cephalosporins. History of anaphylaxis. Concomitant treatment w/ aminoglycosides or diuretics. Renal failure.
Special Precautions
GI disorders. Long-term use. Neonates especially premature infants w/ hyperbilirubinemia. Pregnancy & lactation. Elderly.
Adverse Reactions
Anaphylaxis, thrombophlebitis, erythema multiforme, superinfections, hypoprothrombinemia, hemolytic anemia; thrombocytopenia, eosinophilia, leucopenia, granulocytosis, hallucination, vertigo, disorientation; renal dysfunction; false +ve Coombs' test; increased liver enzymes, serum alkaline phosphatase & transmaminase; biliary sludge or pseudolithiasis; increased BUN, SGOT, SGPT; headache, discomfort, moniliasis, vaginitis, diaphoresis, flushing, leucocytosis, monocytosis, basophilia, jaundice.
