Olanzapine
Indications/Uses
Acute treatment & maintenance of schizophrenia & other psychoses when +ve &/or -ve symptoms are prominent. Moderate to severe manic episode. Prevention of recurrence in patient w/ bipolar disorder.
Dosage/Direction for Use
Schizophrenia & related disorder Starting dose: 10 mg daily. Manic episode Starting dose: 15 mg daily as a single dose in monotherapy or 10 mg daily in combination w/ lithium or valproate. Prevention of recurrence in bipolar disorder Starting dose: 10 mg daily.
Administration
May be taken with or without food.
Contraindications
Narrow-angle glaucoma.
Special Precautions
Hepatic impairment or patients on hepatotoxic drugs, history of seizures, prostatic hypertrophy, paralytic ileus, low leukocyte/neutrophil counts; history of drug-induced bone marrow depression due to concomitant illness, RT or chemotherapy; hypereosinophilic conditions w/ myeloproliferative disease. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Somnolence, wt gain, increase plasma prolactin levels, dizziness, akathisia, increased appetite, peripheral edema, orthostatic hypotension, dry mouth, asymptomatic eosinophilia, constipation, transient increase in liver enzyme levels, NMS, tardive dyskinesia.
Drug Interactions
May antagonize effects of levodopa & dopamine agonists. Induce metabolism w/ smoking, carbamazepine & lorazepam. Reduce bioavailability w/ activated charcoal. Reduced metabolism w/ fluvoxamine, ciprofloxacin or ketoconazole.
