Indications/Uses
Schizophrenia in adult & adolescents (13-17 yr). Manic disorders & mixed episodes associated w/ bipolar 1 disorders as monotherapy & as an adjunct to lithium & valproate therapy.
Dosage/Direction for Use
Schizophrenia Adult Initially 10 or 15 mg once daily. Increment dose should not be administered w/in <2 wk. Adolescents Initially 2 mg, then should be titrated to 5 mg after 2 days later & reach target dose after another 2 days. Bipolar 1 disorder Adult Initially 15 mg given once daily as monotherapy & 10-15 mg given once daily as adjunctive therapy w/ lithium or valproate. Dose may be increased to 30 mg daily based on clinical response. Childn & adolescents (10-17 yr) Recommended starting dose as monotherapy: 2 mg daily, w/ titration to 5 mg daily after 2 days, & a target dose of 10 mg daily after 2 additional days.
Administration
May be taken with or without food: For Schizophrenia: Take w/o meals.
Contraindications
Hypersensitivity.
Special Precautions
Worsening the depression &/or suicidal ideation & behaviour or unusual changes in behavior in adult & ped patients w/ major depressive disorder (MDD). Screening patients for bipolar disorder. NMS may occur. Tardive dyskinesia may develop. May cause orthostatic hypotension, postural dizziness, & syncope in ped patients (6-17 yr). Discontinue treatment in severe neutropenia. Have a history of seizures or w/ conditions that lower the seizure threshold eg, Alzheimer dementia. Increased mortality in elderly w/ dementia-related psychosis.
Adverse Reactions
Headache, nausea, insomnia. Adult: Vomiting, constipation, dizziness, akathisia, anxiety, restlessness. Childn & adolescents: Unconsciousness, extrapyramidal disturbances, fatigue, increased appetite, nasopharyngitis, wt gain.
Drug Interactions
Increased clearance & lower blood levels w/ CYP3A4 inducers eg, carbamazepine. Inhibit elimination & increased blood levels w/ CYP3A4 inhibitors eg, ketoconazole & CYP2D6 inhibitors eg, quinidine, fluoxetine or paroxetine. Avoid alcohol consumption.
