Indications/Uses
Duodenal & gastric ulcers, moderate & severe reflux esophagitis, Zollinger-Ellison syndrome. EC caplet: In combination w/ 2 appropriate antibiotics for the eradication of H. pylori in patients w/ peptic ulcers to reduce recurrence. Other pathological hypersecretory conditions. Powd for inj: Same cases where oral therapy is impossible.
Dosage/Direction for Use
EC caplet H. pylori eradication 40 mg twice daily in combination w/ either 1,000 mg amoxicillin twice daily & 500 mg clarithromycin twice daily or 500 mg metronidazole twice daily & 500 mg clarithromycin twice daily or 1,000 mg amoxicillin twice daily & 500 mg metronidazole twice daily. Gastric & duodenal ulcer, reflux esophagitis 40 mg daily. May increase to 80 mg daily. Zollinger-Ellison syndrome & other pathological hypersecretory conditions Initially 80 mg. May be titrated up or down as needed. Doses >80 mg should be divided & given twice daily. Patient w/ severe liver impairment 40 mg every other day. Patient w/ impaired renal function & elderly Max: 40 mg daily. Powd for inj Gastric & duodenal ulcer, reflux esophagitis 40 mg IV daily. Zollinger-Ellison syndrome & other pathological hypersecretory conditions Initially 80 mg IV. May be titrated up or down as needed. Doses >80 mg should be given twice daily.
Administration
Should be taken on an empty stomach: Take 1 hr before meals. Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. EC caplet: Combination treatment for H. pylori eradication in patients w/ moderate to severe hepatic or renal dysfunction.
Special Precautions
Not indicated for mild GI complaints eg, nervous dyspepsia. Exclude possibility of malignancy of gastric ulcer or a malignant disease of the esophagus prior to treatment. Confirm diagnosis of reflux esophagitis by endoscopy. Pregnancy & lactation. EC caplet: May reduce absorption of vit B12 due to hypo- or achlorhydria in patients w/ Zollinger-Ellison syndrome & other pathological hypersecretory conditions requiring long-term treatment. Monitor prothrombin time/INR in patients treated w/ coumarin anticoagulants. Childn. Powd for inj: Impaired renal function & severe liver impairment (reduce daily dose to 20 mg). Monitor liver enzymes during therapy. Elderly.
Adverse Reactions
Powd for inj: Upper abdominal pain, diarrhea, constipation, flatulence, headache.
Drug Interactions
May reduce absorption of ketoconazole. Powd for inj: Warfarin.
