Estriol
Indications/Uses
Atrophy of lower urogenital tract related to estrogen deficiency, notably for treatment of vag complaints eg, dyspareunia, dryness & itching. Pre- & post-op therapy in postmenopausal women undergoing vag surgery. Climacteric complaints eg, hot flushes & night sweating. Cream: Prevention of recurrent infections of vagina & lower urinary tract, in management of micturation complaints (eg, frequency & dysuria) & mild urinary incontinence; diagnostic aid in case of doubtful atrophic cervical smear.
Dosage/Direction for Use
Tab For urogenital symptoms related to estrogen deficiency 0.5-3 mg/day in a single dose for up to 1 mth, followed by 0.5-1 mg/day until improvement of epithelium occurs (short-term use). Infertility due to cervical hostility 0.5-1 mg/day on days 6-15 of menstrual cycle. Cream Atrophy of the lower urogenital tract 1 application/day for the 1st wks followed by a gradual reduction until maintenance dosage is reached. Pre- & post-op treatment in vag surgery 1 application/day 2 wk pre-op. 1 application twice wkly post-op. Diagnostic aid in case of doubtful atrophic cervical smear 1 application on alternate day in the wk before taking a next smear.
Administration
May be taken with or without food.
Contraindications
Thrombosis, known or suspected estrogen-dependent tumours, undiagnosed vag bleeding. History of manifestations or deterioration of otosclerosis during pregnancy or previous use of steroids. Active thromboembolic disorders, known or suspected breast cancer. Pregnancy.
Special Precautions
A history of latent or overt cardiac failure, fluid retention due to renal dysfunction, HTN, epilepsy or migraine, severe liver disorders, endometriosis, fibrocystic mastopathy, porphyria, hyperlipoproteinaemia, a history during pregnancy of severe pruritus or previous use of steroids, cholestatic jaundice or herpes gestationis. Increases risk of endometrial carcinoma in postmenopausal women. Use w/ caution in gallbladder disorders, hypercalcemia, additional progestin, hypercoagulability, urethral bleeding & mastodynia.
Adverse Reactions
Breast tension or pain, nausea, spotting, fluid retention, cervical hypersecretion, breast enlargement, vag candidiasis, change in vag bleeding pattern, vomiting, stomach cramps, cholestatic jaundice, chloasma or melasma, erythema multiforme, erythema nodosum, hemorrhagic eruption, mental depression, chorea, change in body wt, edema, changes in libido. Rarely headache, HTN, leg cramps, visual disturbances.
