Cefixime trihydrate
Indications/Uses
Uncomplicated UTI caused by E. coli & P. mirabilis; otitis media caused by H. influenzae (β-lactamase +ve & -ve strains), Moraxella (Branhamella) catarrhalis (mostly β-lactamase +ve strains) & Strep pyogenes; acute bronchitis & acute exacerbations of chronic bronchitis caused by Strep pneumoniae & H. influenzae (β-lactamase +ve & -ve strains). Uncomplicated gonorrhoea (cervical/urethral) caused by Neisseria gonorrhoeae (penicillinase- & non-penicillinase-producing strains). Sinusitis caused by susceptible organisms.
Dosage/Direction for Use
Childn 1.5-3 mg/kg twice daily, may be increased up to 6 mg/kg twice daily for severe conditions. Patient w/ renal impairment CrCl 21-60 mL/min or receiving hemodialysis 300 mg daily, CrCl <20 mL/min or receiving peritoneal dialysis & outpatient care 200 mg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity or history of shock.
Special Precautions
Hypersensitivity to penicillin. Discontinue use if symptoms of shock occur (eg, dysphoria, discomfort in the digestive cavity, stridor, headache, abnormal desire for defecation, tinnitus or diaphoresis). Not to be used in patients receiving other antibiotic treatment. Patients w/ hereditary problems eg, bronchial asthma, reddish skin & urticaria; who lack oral nutritional intake, who receive parenteral nutrition. Monitor closely for symptoms of vit K deficiency. Hypersensitivity & shock reactions may occur. Serious kidney dysfunction. Pregnancy & lactation. Elderly or debilitated patients.
Adverse Reactions
Shock; hypersensitivity; granulocytopenia, eosinophilia, thrombocytopenia; increased SGOT, SGPT, alkaline phosphatase; renal insufficiency; severe colitis eg, pseudomembranous colitis (presence of blood in stool), abdominal pain/discomfort, diarrhea, heartburn, anorexia, nausea, satiety, constipation; interstitial pneumonia or PIE syndrome, fever, cough, dysonea, abnormal chest x-ray, eosinophilia; stomatitis, candidiasis; vit K deficiencies eg, hypoprothrombinemia, bleeding tendencies or vit B group deficiencies eg, glossitis, stomatitis, anorexia, neuritis; headache, dizziness.
