Indications/Uses
GERD as an alternative to oral therapy if not appropriate eg, patients w/ esophagitis &/or severe symptoms of reflux; healing of gastric ulcers associated w/ NSAID therapy. Short-term maintenance of haemostasis & prevention of rebleeding in patients following endoscopy for acute gastric bleeding or duodenal ulcers.
Dosage/Direction for Use
Alternative to oral therapy 20-40 mg once daily. Reflux esophagitis 40 mg once daily. Symptomatic treatment of GERD 20 mg once daily. Healing of NSAID-associated gastric ulcers 20 mg once daily. Maintenance of haemostasis & prevention of rebleeding of gastric & duodenal ulcers 80 mg as bolus infusion over 30 min followed by continuous IV infusion of 8 mg/hr over 3 days. Severe hepatic impairment Max: 20 mg daily.
Contraindications
Hypersensitivity to esomeprazole & substituted benzimidazoles. Co-administration w/ atazanavir & nelfinavir is not recommended.
Special Precautions
Avoid concomitant use w/ clopidogrel. Patients at risk for developing osteoprosis or osteoporotic fractures. Exclude gastric malignancy before treatment. Concomitant use w/ drugs metabolized via CYP3A4 eg, cisapride. Regular surveillance in patients on long-term treatment (>1 yr). Pregnancy. Not to be used during breast-feeding.
Adverse Reactions
Headache; abdominal pain, diarrhea, flatulence, nausea, vomiting, constipation. Administration site reactions.
Drug Interactions
Decreased serum levels w/ rifampicin & St. John's wort. Decreased absorption of ketoconazole, itraconazole & erlotinib. Increased absorption of digoxin. Elevated INR w/ warfarin. Increased Cmax & AUC of cilostazol. Increased AUC & prolonged elimination t½ of cisapride. Increased serum levels of tacrolimus & saquinavir. Decreased serum levels of atazanavir & nelfinavir. Decreased clearance of diazepam w/ oral esomeprazole. Increased MTX levels. May result in more than doubling of esomeprazole exposure w/ voriconazole in patients w/ severe hepatic impairment & prolonged use.
