Indications/Uses
Peripheral & central neuropathic pain in adults. Adjunctive therapy in adults w/ partial seizures w/ or w/o secondary generalization. Generalized anxiety disorder & pain in fibromyalgia.
Dosage/Direction for Use
150-600 mg daily in 2 or 3 divided doses. Neuropathic pain Initially 75 mg twice daily, may be increased to 150 mg twice daily after 3-7 days, & further to a max: 300 mg twice daily after an additional wk if needed. Epilepsy Initially 75 mg twice daily, may be increased to 150 mg twice daily after 1 wk, & further to a max: 300 mg twice daily after an additional wk if needed. Generalized anxiety disorder 150-600 mg daily in 2-3 divided doses. Initially 150 mg daily & may be increased to 300 mg daily after 1 wk, then it can be further increased to 450 mg daily on the next following wk. Max: 600 mg daily in the additional wk. Fibromyalgia 300-450 mg daily in 2 divided doses. Initially 75 mg twice daily, may be increased to 150 mg twice daily after 1 wk, then it can be further increased to 225 mg twice daily on the next following wk. Patient w/ renal impairment CrCl ≥60 mL/min 150 mg daily in 2 divided doses. Max: 600 mg daily. CrCl 30-60 mL/min 75 mg daily in 1 or 2 divided doses. Max: 300 mg daily. CrCl 15-30 mL/min 25-50 mg daily in 1 or 2 divided doses. Max: 150 mg daily. CrCl <15 mL/min 25 mg single dose daily. Max: 75 mg daily. Supplementary dose following haemodialysis 25 mg single dose. Max: 100 mg.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue immediately if symptoms of angioedema occur. CHF patients; central neuropathic pain due to spinal cord injury; transient visual blurring & other changes in visual acuity; evaluate patients for history of drug misuse or abuse. Adjust hypoglycemic medications in some diabetic patients who gain wt. W/drawal symptoms after discontinuation of short & long term treatment. Not to be used in patients w/ rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. May influence the ability to drive or use machines. Not to be used during pregnancy. Not recommended during breast-feeding. May increase occurrence of accidental injury (fall) in the elderly.
Adverse Reactions
Dizziness, somnolence. Nasopharyngitis; neutropenia; increased appetite, anorexia, hypoglycemia; psychiatric disorder; nervous system disorder; eye disorder; vertigo, hyperacusis; cardiac disorder; vascular disorder; resp, thoracic & mediastinal disorder; GI disorder; skin & subcutaneous tissue disorder; musculoskeletal & connective tissue disorder; renal & urinary disorder; reproductive system & breast disorder; peripheral oedema, oedema, gait abnormal, fall, feeling drunk, feeling abnormal, fatigue, generalized oedema, chest tightness, pain pyrexial, thirst, chill, asthenia, malaise; increased wt, blood creatine phosphokinase, ALT, AST, blood glucose, blood creatinine; decreased platelet count, blood K, wt, WBC; hypersensitivity, angioedema, allergic reaction.
Drug Interactions
May potentiate effects of ethanol & lorazepam. Additive effects in the impairment of cognitive & gross motor function w/ oxycodone. Resp failure & coma w/ other CNS depressant medications. Reduced lower GI function w/ opioid analgesic.
