Indications/Uses
ER Cap: Control grand mal & psychomotor seizures (lobus temporalis epilepsy). Inj: Control grand mal status epilepticus & prevent seizures during neurosurgery.
Dosage/Direction for Use
Epilepsy ER Cap Adult Initially 100 mg 3 times daily. Maintenance: 300-400 mg daily, increase to 600 mg if necessary. Childn 5 mg/kg daily in 2-3 divided doses. Maintenance: 4-8 mg/kg daily. Max: 300 mg daily. Inj Adult Loading dose: 10-15 mg/kg slow IV for not >50 mg/min. Maintenance dose: 100 mg orally or IV every 6-8 hr. Infant & childn Loading dose: 10-20 mg/kg IV for not >1-3 mg/min. Prevention of seizures during neurosurgery 100-200 mg (1-2 mL) IM w/ 4 hr interval during surgery & continued during the post-op period.
Special Precautions
Avoid abrupt w/drawal. Diabetes, hypotension, serious cardiac insufficiency. May impair ability to drive or operate machinery. Hepatic impairment. Pregnancy & lactation. Elderly or debilitated patients.
Adverse Reactions
Nystagmus, ataxia, slurred speech, mental confusion, dizziness, insomnia, headache, GI disturbances, morbilliform rash, dermatitis, hepatitis, lupus erythematosus, SJS, TEN, thrombocytopenia, leucopenia, granulocytopenia, agranulocytosis, coarse facies, acne & hirsutism, gingival hyperplasia, periarteritis nodosa.
Drug Interactions
Some antibiotics, other anticonvulsants, cimetidine, coumarin anticoagulants, disulfiram, INH, some phenothiazines, phenylbutazone, sulfinpyrazone, carbamazepine. Avoid use w/ alcohol.
