Methylprednisolone
Indications/Uses
Adrenocortical insufficiency, congenital adrenal hyperplasia, non-suppurative thyroid disease, hyperkalemia associated w/ cancer, allergies, edema, acute exacerbations of multiple sclerosis, GI disorders, certain skin & resp tract inflammations, haematological, collagen, & rheumatic diseases.
Dosage/Direction for Use
Adult Initially 4-48 mg daily as a single or divided doses, depending on the condition of the disease & patient's response. Maintenance: Lower dose is required. Multiple sclerosis 160 mg daily for 1 wk followed by 64 mg daily for 1 mth.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Systemic fungal infections.
Special Precautions
Long-term use can cause subcapsular cataracts, glaucoma, secondary ocular infections, associated w/ fungi & viruses, & reduce the body's resistance to infection. High doses may cause decreased BP, salt & water retention, & increased K & Ca excretion. Patients receiving corticosteroid therapy should not be vaccinated against smallpox; receiving high doses should not be given other immunizations. Monitor for disease reactivation in patients w/ latent TB or tuberculin reactivity. May increase corticosteroid effects in patients w/ cirrhosis & hypothyroidism. Not recommended in pregnant & lactating women. Not recommended in infants, childn. Long-term use in elderly.
Adverse Reactions
Body fluids & electrolytes disturbances, cataracts, muscle weakness, aseptic necrosis, osteoporosis, pathological fractures, impaired wound healing, impaired growth in childn, increased BP, peptic ulcer w/ possible perforation & bleeding, pancreatitis, ulcerated oesophagitis, stomach perforation, gastric bleeding, abdominal bloating, increased ICP, pseudotumor cerebri & epilepsy.
Drug Interactions
Concomitant use w/ cyclosporine increases the inhibitory effect of metabolism & occurrence of convulsions. Concomitant use w/ aspirin increases the chronic clearance of high-dose aspirin. Reduced anticoagulant effects w/ various anticoagulants. Increased methylprednisolone clearance w/ drugs that induce hepatic enzymes (phenobarb, phenytoin, rifampicin, rifabutin, carbamazepine, pyrimidine, & aminoglutethimide). Inhibited methylprednisolone metabolism & clearance w/ troleandomycin & ketoconazole. Neuromuscular blocking agents. Reduced anticholinesterase effect in myasthenia gravis.
