Hyoscine butylbromide
Indications/Uses
Sugar-coated tab: GIT & GUT spasm, spasm & dyskinesia of the biliary system. Inj: Acute GI, biliary & genitourinary spasm including biliary & renal colic. Aid in diagnostic & therapeutic procedures where spasm may be a problem eg, gastroduodenal endoscopy & in radiology.
Dosage/Direction for Use
Patient >12 yr Sugar-coated tab 1-2 tab 3 or 4 times daily. Inj 1-2 amp (20-40 mg) by slow IV or IM inj several times daily. Max: 100 mg daily.
Administration
May be taken with or without food: Swallow tab whole w/ sufficient liqd.
Contraindications
Hypersensitivity. Mechanical stenosis of GIT, tachycardia, megacolon, myasthenia gravis. Sugar-coated tab: Urinary retention w/ subvesical obstruction (eg, benign prostatic hyperplasia), narrow-angle glaucoma, tachyarrhythmia. Inj: Untreated narrow-angle glaucoma, hypertrophy of the prostate w/ urinary retention, paralytic or obstructive ileus. Patients being treated w/ anticoagulant drug since IM haematoma may occur (SC or IV route may be used instead).
Special Precautions
Severe & unexplained abdominal pain persist or worsen, or occur together w/ symptoms eg, fever, nausea, vomiting, change in bowel behavior, abdominal tenderness, decreased BP, fainting or blood in the stool. Increase in IOP in patients w/ untreated narrow angle glaucoma. Avoid during pregnancy & breastfeeding. Sugar-coated tab: Thyrotoxicosis, cardiac insufficiency or cardiac surgery failure. Patients prone to intestinal or urinary tract obstruction. Pyrexia patients. Inj: Cases of anaphylaxis including episodes of shock have been observed. Not to be given continuously for a long time w/o investigating the cause of abdominal pain. Observe patients w/ heart disease. Caution when driving a car or operating machinery.
Adverse Reactions
Skin reactions eg, urticaria, rash, erythema, pruritus, & other hypersensitivity; tachycardia; dizziness; dry mouth (inhibition of salivary secretion); dyshidrosis. Sugar-coated tab: Drowsiness; diarrhea, nausea, vomiting, stomach pain; inhibition of sweat secretion. Inj: Anaphylactic shock including fatal outcome, anaphylactic reactions, dyspnoea; accommodation disorders, mydriasis, increased ocular HTN; decreased BP, flushing; urinary retention.
Drug Interactions
Anticholinergic effect of drugs eg, tri- & tetracyclic antidepressants, antihistamines, antipsychotics, quinidine, amantadine, disopyramide & other anticholinergics (eg, tiotropium, ipratropium, atropine-like compd) may be intensified by hyoscine butylbromide. Co-administration w/ dopamine antagonists eg, metoclopramide will exert a mutually neutralizing effect on the GIT. Tachycardiac effect of β-adrenergic drugs may be enhanced by hyoscine butylbromide.
