Cefotaxime
Indications/Uses
Lower resp tract, skin, skin structure & soft tissue, bone & joint & intra-abdominal infections; UTI & gynecological infections; meningitis; septicemia; bacteremia. Post-op infection prophylaxis.
Dosage/Direction for Use
Adult & childn >12 yr 2-6 g daily. Max: 12 g daily. Mild to moderate or uncomplicated infections 1 g 12 hrly. Moderate to severe infections 1 g 8 hrly. Life-threatening infections 2 g 8 hrly. Gonorrhea 1 g IM/IV as a single dose. Childn & infant 1 mth-12 yr 50-180 mg/kg IM/IV in 4-6 equally divided doses. Childn ≥50 kg Adult dose. Max: 12 g daily. Neonate & premature infant 1-4 wk 50 mg/kg 8 hrly; 0-1 wk 50 mg/kg IV 12 hrly. Prophylaxis of post-op infections 1 g IM/IV as a single dose 30-90 min pre-op. Caesarian section 1st dose: 1 g IV immediately after umbilical cord is clamped; 2nd dose: 1 g IV/IM 6 hr after 1st dose; 3rd dose: 1 g IV/IM 12 hr after 1st dose.
Contraindications
Hypersensitivity to cephalosporins.
Special Precautions
May cause cross-sensitivity reactions in patients w/ hypersensitivity to penicillin. History of GI disease especially colitis. Blood count should be performed in prolonged treatment (>10 days). Renal impairment. Pregnancy & lactation.
Adverse Reactions
Local inj site reactions, rash, pruritus, fever, eosinophilia, urticaria, anaphylaxis; colitis, diarrhea, nausea, vomiting; neutropenia, leucopenia, thrombocytopenia, agranulocytosis; moniliasis, vaginitis; headache; transient increased SGOT/SGPT/LDH/alkaline phosphatase; interstitial nephritis, transient BUN & creatinine elevation; erythema multiforme, SJS, TEN.
Drug Interactions
Probenecid, aminoglycosides; potent diuretics.
