Metformin HCl
Indications/Uses
As monotherapy or in combination w/ sulfonylurea in patients w/ NIDDM & over wt if blood glucose cannot be controlled by diet alone. Adjuvant therapy in hardly controlled IDDM.
Dosage/Direction for Use
Adult Initially 850 mg twice daily or 500 mg 3 times daily, may be increased to max of 3 g daily. Combination w/ sulfonylurea 500 mg or ½ 850 mg caplet daily, may slowly increase the dose until optimal control. May be continued w/ metformin alone. Combination w/ insulin Insulin dosage <60 u daily Initially 500 mg or ½ 850 mg caplet daily while gradually decreasing insulin dosage 4 u every 2-4 day. Dosage may be increased at wkly interval.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Diabetic coma & ketoacidosis. Chronic liver disease, heart failure & MI, alcoholism, history of lactic acidosis (eg, shock or pulmonary insufficiency & conditions related to hypoxemia). Severe kidney dysfunction. Pregnancy & lactation.
Special Precautions
Patient w/ dehydration or recovering from serious infections or trauma. Periodically check vit B12 levels on long-term use; monitor blood sugar levels. Clinically monitor patient for stabilization if treatment is combined w/ insulin. Renal impairment. Childn. Elderly w/ renal impairment.
Adverse Reactions
GI disturbance; intolerance; anorexia, nausea, vomiting, diarrhea; reduced vit B12 absorption.
Drug Interactions
Anticoagulants. Reduced renal clearance w/ cimetidine.
