Hyoscine butylbromide
Indications/Uses
Acute GI, biliary & genitourinary spasm, including biliary & renal colic. Aid in diagnostic & therapeutic procedures, where spasm may be a problem eg, gastroduodenal endoscopy, & in radiology.
Dosage/Direction for Use
Adult & adolescents >12 yr 1-2 amp (20-40 mg) may be administered by slow IV, IM inj several times daily. Max: 100 mg daily.
Contraindications
Hypersensitivity. Untreated narrow angle glaucoma; hypertrophy of the prostate w/ urinary retention; mechanical stenosis in the GIT; tachycardia; paralytic or obstructive ileus; megacolon; myasthenia gravis. IM inj: Patients being treated w/ anticoagulants since IM haematoma may occur.
Special Precautions
Investigate the etiology of severe, unexplained abdominal pain that persist or worsens, or occurs together w/ symptoms eg, fever, nausea, vomiting, changes in bowel movements, abdominal tenderness, decreased BP, fainting or blood in the stool w/ appropriate diagnostic measures. Patients w/ heart disease. Elevation of IOP in patients w/ undiagnosed & untreated narrow angle glaucoma. Seek urgent ophth advice in case a painful, red eye w/ loss of vision after inj develops. Anaphylaxis including episodes of shock after inj; observe patients receiving treatment by inj. Avoid potentially hazardous tasks eg, driving or operating machinery if accommodation disorder or dizziness occurs. Avoid use during pregnancy & lactation.
Adverse Reactions
Anaphylactic shock including fatal outcome, anaphylactic reactions, dyspnoea, skin reactions (eg, urticaria, rash, erythema, pruritus) & other hypersensitivity; accommodation disorders, mydriasis, increased ocular HTN; tachycardia; decreased BP, dizziness, flushing; dry mouth; dyshidrosis; urinary retention.
Drug Interactions
Intensified anticholinergic effects of tri- & tetracyclic antidepressants, antihistamines, antipsychotics, quinidine, amantadine, disopyramide & other anticholinergics (eg, tiotropium, ipratropium, atropine like compd). Concomitant treatment w/ dopamine antagonists eg, metoclopramide may result in diminution of the effects of both drugs on the GIT. Enhanced tachycardiac effects of β-adrenergic agents.
