Filgrastim (recombinant human granulocyte colony stimulating factor)
Indications/Uses
Reduction in the duration of neutropenia in patients treated w/ established cytotoxic chemotherapy for non-myeloid malignancy.
Dosage/Direction for Use
Recommended dose: 5 mcg/kg daily SC bolus or short IV infusion, given over 30 min. Should be administered no earlier than 24 hr after the administration of cytotoxic chemotherapy & not be administered in the period 24 hr before the administration of chemotherapy.
Contraindications
Unknown hypersensitivity to E. coli-derived proteins, filgastrim. Myeloid malignancy. Not to be used to increase the dose of cytotoxic chemotherapy beyond dosage regimens.
Special Precautions
Discontinue immediately if leucocyte counts exceed 50 x 109/L. Not recommended in myeloid premalignant & malignant condition. Perform WBC count at regular intervals during therapy; CBC is recommended twice wkly during therapy. Monitor bone density in patients w/ underlying osteoporotic bone disease who received Filgrastim. Avoid simultaneous use of Filgrastim w/ chemotherapy & RT. Not recommended in patients w/ severe renal or hepatic function impairment. Pregnancy & lactation. Childn.
Adverse Reactions
Mild to moderate musculoskeletal pain during treatment (severe cases occasionally appear), dysuria, thrombocytopenia, splenomegaly, anemia, epistaxis, cephalgia, diarrhea,vasculitis, medullary bone pain, nausea/vomiting, alopecia, neutropenic fever, mucositis, fatigue, anorexia, dyspnea, headache, cough, skin rash, chest pain, stomatitis, constipation, pain, & reversible elevation in serum uric acid, LDH enzyme, alkaline phosphatase, γ-glutamyl transpeptidase.
Drug Interactions
Not to be diluted w/ saline soln. Lithium may potentiate the release of neutrophils.
