Piroxicam
Indications/Uses
Caplet: Symptomatic treatment of RA, OA, ankylosing spondylitis, acute musculoskeletal disorders, acute gout. Gel: Pain w/ inflammation eg, OA, post-traumatic or musculoskeletal disorders, including tendinitis, tenosynovitis, periarthritis, sprains, strains, back pain.
Dosage/Direction for Use
Caplet Adult RA, OA, ankylosing spondylitis Initially 20 mg as a single dose. Maintenance dose: 20 mg daily, may be given 10-30 mg as a single or divided dose. Doses >20 mg daily may increase GI side effects. Acute gout Initially 40 mg daily as a single dose, then followed by 40 mg daily as a single or divided doses 4-6 days later. Acute musculoskeletal disorders 40 mg as a single or divided dose for 2 days, then 20 mg daily for 7-14 days. Gel Apply 1 g gel on the affected area 3-4 times daily. Rub slowly.
Administration
Should be taken with food: Take immediately after meals.
Contraindications
Hypersensitivity. Patients who have bronchospasm, nasal polyp & angioedema or urticaria after taking acetosal or other NSAIDs. A history of gastric ulcer or bleeding.
Special Precautions
Pregnancy, lactation. Caplet: Patients w/ existing or risk factor of CV disease, GI disorders, kidney or liver disease, HTN, fluid retention, or heart failure. Patients w/ a history of GI ulcer or bleeding. Patients w/ predisposing factor of blood coagulation disorders. May cause liver damage, increased SGPT/SGOT & jaundice. Concomitant use w/ acetosal. Childn. Gel: Discontinue treatment if local irritation is getting worse. Not for ophth, mucous membrane or open wound.
Adverse Reactions
Caplet: Stomatitis, anorexia, epigastric distress, nausea, constipation, abdominal discomfort, bloating, diarrhea, abdominal pain. Gastric bleeding, perforation & ulcer. Edema, dizziness, headache, skin rash, pruritus, somnolence, Hb, hematocrit. Gel: Erythema, rash, pityroid desquamation, pruritus, local reactions.
Drug Interactions
Caplet: Oral anticoagulants, sulphonylurea, hydantoin, acetosal, lithium.
