Digoxin
Indications/Uses
CHF; paroxysmal supraventricular tachycardia. Inj: Heart failure w/ atrial fibrillation.
Dosage/Direction for Use
Rapid digitalization (24-36 hr) Adult 4-6 tab followed at adequate interval by 1 tab until compensation is achieved. Childn 25 mcg/kg, increasing at interval until compensation achieved. Maintenance: 10-20 mcg/kg daily. Slow digitalization (3-5 days) 2-3 tab daily in divided doses. Maintenance: 1-3 tab daily. Inj 0.5-1 mg/day.
Administration
May be taken with or without food.
Contraindications
Ventricular tachycardia & fibrillation, 2nd degree & complete AV block, sinus arrest, excessive sinus bradycardia.
Special Precautions
Partial heart block, acute myocarditis, rheumatic carditis. Impaired renal function. Pregnancy.
Adverse Reactions
CNS & GI disturbances. Rarely, confusion, disorientation, aphasia, disturbances of heart rate, conduction & rhythm. Exceptionally, allergic skin reactions, gynecomastia.
Drug Interactions
Amphotericin & drugs which deplete K may enhance possibility of digoxin toxicity. Absorption may be inhibited by antacids; cholestyramine, colestipol, neomycin, sulfasalazine. Increased risk of cardiac arrhythmia w/ Ca salts & antiarrhythmics. Serum conc may be increased by quinidine.
