Indications/Uses
Advanced breast cancer in women w/ natural or induced post-menopausal status whose disease has progressed following anti-oestrogen therapy. Adjuvant treatment of post-menopausal women w/ estrogen-receptor +ve early breast cancer, following 2-3 yr of initial adjuvant tamoxifen therapy.
Dosage/Direction for Use
Elderly & adult 1 tab once daily. Patient w/ early breast cancer Continue treatment until completion of 5 yr of adjuvant endocrine therapy, or until local or distant recurrence or new contralateral breast cancer.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Pre-menopausal women. Pregnancy & lactation.
Special Precautions
Not to be administered to women w/ pre-menopausal endocrine status. Reductions in bone mineral density. Women w/ vit D deficiency should receive vit D supplementation. Assess LH, FSH & oestradiol levels to ascertain post-menopausal status; bone mineral density of women w/ osteoporosis or at risk of osteoporosis at commencement of treatment. Consider routine assessment of 25-hydroxy vit D levels prior to treatment. May impair ability to drive or operate machinery. Hepatic or renal impairment. Not be administered to lactating woman.
Adverse Reactions
Depression, insomnia; headache, dizziness; hot flushes; abdominal pain, nausea; increased hepatic enzyme, blood bilirubin & blood alkaline phosphatase; increased sweating; joint & musculoskeletal pain; pain, fatigue. Anorexia; carpal tunnel syndrome; vomiting, diarrhea, constipation, dyspepsia; alopecia, rash; peripheral oedema, asthenia. Paraesthesia; urticaria, pruritus.
Drug Interactions
Concomitant use w/ drugs metabolised via CYP3A4 & w/ narrow therapeutic window. Negated pharmacological action w/ oesterogen-containing medicines. Reduced efficacy w/ rifampicin, anticonvulsants (eg, phenytoin & carbamazepine) & herbal prep containing Hypericum perforatum (St. John's Wort).
