Duloxetine
Indications/Uses
Depression. Diabetic peripheral neuropathic pain (DPNP). Treatment of generalized anxiety disorder (GAD) & prevention of recurrence for 6 mth.
Dosage/Direction for Use
Depression & DPNP Initially 60 mg once daily. Max: 120 mg daily in 2 divided doses. GAD Initially 30 mg once daily, may increase to 60 mg if insufficient response.
Administration
May be taken with or without food.
Contraindications
Concomitant use w/ non-selective irreversible MAOIs & potent CYP1A2 inhibitors eg, fluvoxamine, ciprofloxacin, enoxacin. Hepatic & severe renal (CrCl <30 mL/min) impairment.
Special Precautions
Suicidal tendency in bipolar depression. Activation of mania/hypomania. Seizure, mydriasis. Substantial alcohol use. Monitor BP. Rare heredity problems of fructose intolerance, glucose-galactose malabsorption or sucrose-isomaltose insufficiency. May impair ability to drive & operate machinery. Renal or hepatic impairment. Pregnancy & lactation. Monitor in childn & adolescents <18 yr.
Adverse Reactions
Nausea, headache, dry mouth, fatigue, somnolence, insomnia, dizziness, constipation, diarrhea, decreased appetite, hyperhydrosis.
Drug Interactions
Drugs metabolised by & inhibitors of CYP1A2 & CYP2D6. CNS drugs. SSRIs, serotonin & norepinephrine reuptake inhibitors, triptans, tramadol, highly protein-bound drugs, steroids, MAOIs.
