Tizanidine HCl
Indications/Uses
Painful muscle spasm associated w/ functional & static spinal cord disorders (cervical & lumbar syndrome); post-op painful muscle spasm ie, herniated intervertebral disc or hip OA. Spasticity due to neurological disorders ie, multiple sclerosis, chronic myelopathy, degenerative spinal cord disorders, CVA, cerebral palsy.
Dosage/Direction for Use
Painful muscle spasm Initially 2-4 mg 3 times daily. Severe cases: Extra dose of 2-4 mg may be taken at night. Spasticity due to neurologic disorder Max initial dose: 6 mg daily in 3 divided doses. May be gradually increased to 2-4 mg w/ 1 wkly interval. Max: 36 mg daily.
Contraindications
Hypersensitivity. Concomitant use w/ potent CYP1A2 inhibitor eg, fluvoxamine or ciprofloxacin. Severe liver dysfunction. Pregnancy.
Special Precautions
Avoid abrupt w/drawal. Smoking. May impair the ability to drive or operate the machinery. Patients w/ kidney or liver dysfunction need dose adjustment. Pregnancy or any potency of pregnancy. Lactation.
Adverse Reactions
Dizziness, GI disturbances, dry mouth, fatigue, nausea. Low dose: Drowsiness, reduced BP, increased transaminase. High dose: Hypotension, bradycardia, muscle weakness, insomnia, sleep disturbances, hallucination, hepatitis. Mental confusion; vertigo; syncope; weakness, w/drawal syndrome; visual disturbances; liver failure.
Drug Interactions
Fluvoxamine, ciprofloxacin, antiarrhythmics (eg, amiodarone, mexiletine, propafenone), cimetidine, fluoroquinolones (enoxacin, pefloxacin, norfloxacin), rofecoxib, OC, ticlopidine; drugs which may prolong QT interval (eg, cisapride, amitriptyline, azithromycin); antihypertensives, including diuretics; rifampicin; alcohol; hypnotic sedative drugs (eg, benzodiazepine or baclofen), other antihistamines, α-2 adrenergic agonists (ie, clonidine).
