Methylprednisolone acetate
Indications/Uses
Endocrine, rheumatic & haematologic disorders; collagen, dermatologic, ophth, GI, resp & neoplastic diseases, allergic & edematous states; acute exacerbation of multiple sclerosis; tuberculous meningitis w/ subarachnoid or impending block when used concurrently w/ appropriate anti-TB chemotherapy; trichinosis w/ neurologic or myocardial involvement. As adjunctive therapy for short term intra-synovial administration in acute episode or exacerbation. Intralesional administration in keloids, localized hypertrophic, infiltrated & inflammatory lesions.
Dosage/Direction for Use
RA & OA Large joint: 20-80 mg. Medium joint: 10-40 mg. Small joint: 4-10 mg. By IA administration for local effect & IM for systemic effect.
Contraindications
Hypersensitivity. Systemic fungal infection. Do not inj IV, epidural or intrathecally. Use w/ live or live attenuated vaccines.
Special Precautions
Unusual stress, intercurrent infection; immunisation (defer); active or latent TB; ocular herpes simplex; renal insufficiency, HTN; osteoporosis; myasthenia gravis. Non-specific ulcerative colitis. Not for intrathecal administration. Pregnancy.
Adverse Reactions
Local atrophy, pigmentation changes, post-inj flare, sterile abscess, Charcot-like syndrome; Na & fluid retention; impaired wound healing, thin fragile skin; decreased carbohydrate tolerance; muscle weakness, myopathy, aseptic necrosis, tendon rupture; cataract, increased IOP & ICP; growth retardation; psychic derangements.
Drug Interactions
Mutual inhibition of metabolism of methylprednisolone w/ cyclosporine. Increased methylprednisolone clearance w/ phenytoin, phenobarb, rifampicin. Inhibit metabolism & decrease clearance of methylprednisolone w/ troleandomycin & ketoconazole. Methylprednisolone may increase the clearnace of chronic high-dose aspirin & decrease salicylate levels. Corticosteroids may enhance as well as diminish effect of anticoagulant.
