Clomiphene citrate
Indications/Uses
Female infertility due to ovulation disorders. Male infertility due to oligospermia.
Dosage/Direction for Use
Anovulatory infertility in women 1 tab daily for 5 days starting on the 5th day of menstrual cycle or any time in amenorrhea. Ovulation usually occurs w/in 6-10 days after last dose. If no ovulation occurs, may increase dose to 100 mg daily as a single dose for 5 days, starting as early as 30 days from the 1st administration. Oligospermia 1 tab daily for 40-90 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Organic disorders of the pituitary, ovary (eg, abnormal uterine bleeding, ovarian endometriosis) or other reproductive organs; endometrial carcinoma; bilirubin metabolism disorder; uncontrolled adrenal or thyroid dysfunction; inorganic intracranial lesion (eg, pituitary tumors). Hepatic insufficiency. Pregnancy & ovarian cysts.
Special Precautions
Conduct accurate O & G exam prior to initiation of treatment. Possibility of multiple pregnancies must be made known to the patient. Discontinue treatment in the occurrence of visual impairment eg, blurred vision or presence of spots or flashes; rarely, scotomata can occur. Treat infertility due to endocrine disorders first prior to therapy. Conduct semen analysis in infertile couples who are undergoing ovulation induction therapy. Conduct close evaluation including pelvic exam prior to therapy. Dose reduction may be needed to reduce risk of ovarian enlargement. Ovarian hyperstimulation syndrome (abdominal pain, GI disorders, ovarian enlargement, wt gain, dyspnea, oliguria) may occur in patients using clomiphene citrate for ovulation induction therapy. Long term use may increase risk of ovarian tumors. Treatment >5 days is not recommended.
Adverse Reactions
Ovarian & abdominal enlargement, pelvic discomfort, vasomotor symptoms eg, hot flushes; nausea, vomiting, breast pain & visual disturbances; restlessness, insomnia, headache, dizziness, increased urine production, heavy menstruation, depression, fatigue, skin reactions eg, dermatitis, urticaria; seizures; multiple pregnancy.
