Doripenem
Indications/Uses
Nosocomial pneumonia (including ventilator-associated pneumonia); complicated intra-abdominal infections.
Dosage/Direction for Use
Nosocomial pneumonia (including ventilator-associated pneumonia) 500 mg every 8 hr infused in 1 or 4 hr. Complicated intra-abdominal infection 500 mg every 8 hr infused in 1 hr. Treatment duration: 5-14 days. Patient w/ severe renal impairment (CrCl <30 mL/min) 250 mg every 12 hr. Patient w/ moderate renal impairment (CrCl 30-50 mL/min) 250 mg every 8 hr.
Contraindications
Hypersensitivity to doripenem, any other carbapenem antibacterial agent or any other β-lactam antibacterial agent eg, penicillins or cephalosporins.
Special Precautions
Hypersensitivity reactions, seizures, pseudomembranous colitis, superinfection. Prolonged use should be avoided. Pneumonitis. Pregnancy & lactation.
Adverse Reactions
Headache; diarrhea, nausea. Oral candidiasis, vulvomycotic infection; hypersensitivity reactions; phlebitis; colitis caused by C. difficile; increased hepatic enzyme; pruritus, rash.
Drug Interactions
May reduce serum valproic acid conc. Reduced renal clearance & increased AUC w/ probenecid.
