Indications/Uses
HTN. Heart failure & left ventricular systolic function impairment (LVEF ≤40%) when ACE inhibitors are not tolerated.
Dosage/Direction for Use
HTN Adult Initially 4 mg daily. May be increased up to a max of 16 mg daily. Patient w/ moderate to severe renal failure, mild to moderate liver failure & elderly Initially 2 mg once daily. May be increased according to patient's response. Heart failure Adult Initially 4 mg daily. May be increased up to 32 mg once daily or higher doses that the patient can tolerate. May double the dose w/ a min interval of 2 wk.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Severe liver damage &/or cholestasis. Pregnancy & lactation.
Special Precautions
Monitor BP, K & serum creatinine levels. Concomitant use w/ ACE inhibitors. Patients w/ bilateral renal artery stenosis or one kidney artery stenosis; aortic & mitral valve stenosis; obstructive hypertrophic cardiomyopathy. Hemodialysis patients. Treat hypovolemia first prior to therapy. Hypotension; hyperkalemia. Not recommended in patients w/ primary hyperaldosteronism. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Severe renal failure.
Adverse Reactions
Dizziness, vertigo, headache; resp infections; hyperkalemia; kidney disorders; leukopenia, neutropenia, agranulocytosis; hyperkalemia, hyponatremia; dizziness, headache; nausea; increased liver enzymes, abnormal liver function or hepatitis; angioedema, reddish skin, urticaria, pruritus; back pain, arthralgia, myalgia; kidney disorders, kidney failure in certain patients.
