Indications/Uses
Treatment of herpes zoster (shingles) & ophth zoster in immunocompetent adult; herpes zoster in adult w/ mild or moderate immunosuppression. Treatment of 1st-episode & recurrences of genital herpes in immunocompetent adult & adolescent & in immunocompromised adult. Suppression of recurrent genital herpes in immunocompetent adult & adolescent & in immunocompromised adult. Treatment & suppression of recurrent ocular HSV infections in immunocompetent adult & adolescent & in immunocompromised adult. Prophylaxis of cytomegalovirus (CMV) infection & disease following solid organ transplantation in adult & adolescent.
Dosage/Direction for Use
Herpes & ophth zoster Immunocompetent adult 1,000 mg tds for 7 days. Immunocompromised adult 1,000 mg tds for at least 7 days & for 2 days following crusting of lesions. HSV infection Immunocompetent adult & adolescent ≥12 yr 500 mg bd for 3-5 days for recurrent episodes. Treatment may be extended to 10 days for initial, more severe episodes. Immunocompromised adult 1,000 mg bd for at least 5 days; may have to be extended to 10 days for initial, more severe episodes. Herpes labialis 2,000 mg bd for 1 day. Take 2nd dose about 12 hr after the 1st dose. Suppression of recurrence of HSV infection Immunocompetent adult & adolescent ≥12 yr 500 mg once daily. Immunocompromised adult 500 mg bd. Prophylaxis of CMV infection & disease 2,000 mg qds. Renal impairment Reduce dose according to CrCl.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to valaciclovir, aciclovir or excipients.
Special Precautions
Ensure adequate fluid intake particularly in elderly. Consider IV antiviral therapy when response to oral therapy is considered insufficient. Risk of transmission of genital herpes; CNS abnormalities in CMV-infected patients receiving high dose valaciclovir. Renal & hepatic impairment. Liver transplant patients. Pregnancy & lactation. Elderly.
Adverse Reactions
Headache. Nausea; dizziness; vomiting, diarrhoea; rashes including photosensitivity, pruritus.
Drug Interactions
Increased nephrotoxicity w/ aminoglycosides, organoplatinum compd, iodinated contrast media, methotrexate, pentamidine, foscarnet, ciclosporin & tacrolimus. Increased AUC w/ cimetidine, probenecid, tenofovir, mycophenolate mofetil.
