Indications/Uses
Management of renal tubular acidosis w/ Ca stones; hypocitraturic Ca oxalate nephrolithiasis of any etiology; uric acid lithiasis w/ or w/o Ca stones.
Dosage/Direction for Use
Severe hypocitraturia Initiate w/ 60 mEq/day (30 mEq bd or 20 mEq tds). Mild to moderate hypocitraturia Initiate w/ 30 mEq/day (15 mEq bd or 10 mEq tds).
Administration
Should be taken with food: Take immediately after meals.
Contraindications
Patients w/ hyperkalemia or conditions predisposing to hyperkalemia (eg, chronic renal failure, uncontrolled DM, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown or administration of a K-sparing agent); patients in whom there is cause for arrest or delay in tab passage through the GI tract eg, delayed gastric emptying, esophageal compression, intestinal obstruction or stricture, or those taking anticholinergic medication; patients w/ peptic ulcer disease; active UTI (w/ either urea-splitting or other organisms, in association w/ either Ca or struvite stones); renal insufficiency (GFR <0.7 mL/kg/min).
Special Precautions
Risk of hyperkalemia & cardiac arrest in patients w/ impaired mechanisms for excreting K. Reports of upper GI mucosal lesions. Pregnancy & lactation. Childn.
Adverse Reactions
Abdominal discomfort, vomiting, diarrhoea, loose bowel movements, nausea.
Drug Interactions
Risk of severe hyperkalemia w/ K-sparing diuretics. Increased GI irritation produced by K salts w/ anticholinergics & other drugs which delay GI transit time.
