Tetracycline (as lymecycline)
Indications/Uses
Treatment of Brucella infection; Pasteurella infection; pulmonary, genito-urinary & ophth infections due to Chlamydiae; pulmonary & genito-urinary infections due to Mycoplasma; Rickettsiae infection; Coxiella burnetti (Q fever), Gonococci infection; resp, broncho-pulmonary infections due to H. influenzae, particularly when there is an acute exacerbation of chronic bronchitis; Treponema (tetracyclines are indicated in syphilis only when the patient is allergic to β-lactamines); Spirochetes (lyme disease, leptospirosis); cholera; acne (cutaneous manifestations in relation w/ Propionibacterium acnes).
Dosage/Direction for Use
Adult Chronic treatment of acne 1 cap daily (300 mg daily), continued for at least 8 wk. Other infections 1 cap bd (600 mg daily). If higher doses are required, 3-4 cap (900-1,200 mg) may be given over 24 hr. Lower doses may be given for prophylaxis. Childn >12 yr Adult dose may be given.
Contraindications
Hypersensitivity to lymecycline or to other tetracyclines. Concurrent treatment w/ retinoids. Childn <8 yr.
Special Precautions
May cause oesophageal irritation & ulceration. Take Tetralysal w/ adequate fluids (water). Overdosage could result in hepatotoxicity. Risk of photosensitivity. Avoid exposure to direct sunlight & UV light during treatment. Caution in patients w/ renal or hepatic impairment. May cause false +ve urine glucose determinations. May interfere w/ fluorometric determinations of urine catecholamines resulting in falsely increased values. Should not be administered to pregnant or breast-feeding women. Safety & efficacy in childn <12 yr have not been established.
Adverse Reactions
Nausea, abdominal pain, diarrhoea; headache.
Drug Interactions
Risk of intracranial HTN w/ oral retinoids & vit A (>10,000 IU daily). Decreased absorption w/ Fe prep & antacids, Mg/Al & Ca hydroxides, oxides, salts, cholestyramine, bismuth chelates, sucralfate, quinapril; medicinal products which increase gastric pH. Decreased t½ w/ enzyme inducers eg, barbiturates, carbamazepine, phenytoin. Increased effect of oral coumarin-type anticoagulants. May interfere w/ the bactericidal action of penicillins & β-lactam antibiotics. Reports of fatal renal toxicity w/ methoxyflurane. Increased serum levels of lithium.
