Clostridium botulinum toxin type A
Indications/Uses
Treatment of benign essential blepharospasm in patients ≥18 yr; equinus foot deformity due to spasticity in ped cerebral palsy patients ≥2 yr; upper limb spasticity associated w/ stroke in patient ≥20 yr. Temporary improvement of serious glabellar wrinkles ranging from moderate to severe associated w/ corrugators muscle &/or procerus muscle activities in adults >20 & <65 yr.
Dosage/Direction for Use
Blepharospasm Initially 1.25-2.5 u (0.05- 0.1 mL at each inj site) injected into the medial & lateral pre-tarsal orbicularis oculi of the upper lid & into the lateral pre-tarsal orbicularis oculi of the lower lid. Treatment can be repeated after approx 3 mth. At repeat treatment sessions, dose may be increased up to 2-fold if there is insufficient response from initial treatment. Max cumulative dose in a 30-day period: 200 u. Ped cerebral palsy Inj to each of the medial & lateral heads of the gastrocnemius muscles. Max 200 u/patient at a time. Patient w/ hemiplegia Total dose of 4 u/kg in the affected gastrocnemius muscle. Patient w/ diplegia 6 u/kg divided between both legs. Glabellar wrinkles 20 u injected to 2 places on the corrugators muscle for each eye & 1 place on the procerus muscle. Total of 5 sites w/ 0.1 mL/site. Muscle spasticity: Biceps brachii 100-200 u, up to 4 sites; flexor digitorum profundus 15-50 u, 1-2 sites; flexor digitorum sublimis 15-50 u, 1-2 sites; flexor carpi radialis 15-60 u, 1-2 sites; flexor carpi ulnaris 10-50 u, 1-2 sites.
Special Precautions
Blepharospasm: Some tolerance may develop when used more frequently than every 3 mth. Ped cerebral palsy: After inj, monitor patient for at least 30 min for any acute adverse event. Glabellar wrinkles: Avoid inj in the levator palpebrae superioris vicinity to reduce complications of ptosis especially for patients w/ large corrugators muscles. Inj in a place at least 1 cm apart from supraorbital ridge when injecting in the medial end of corrugators muscle & in the midpoint between each eyebrow. Exercise caution when injecting to avoid entering blood vessels & effusion from the area below the orbital ridge. Frequent inj is not recommended. 1st Siax inj induces chemical denervation in the injected muscles 1-2 days after inj & its intensity increases during the 1st wk.
