Rivastigmine
Indications/Uses
Adult patients w/ mild to moderately severe Alzheimer's dementia & mild to moderately severe dementia associated w/ Parkinson's disease.
Dosage/Direction for Use
Individualized dosage. Start w/ a low dose, then slowly increase depending on treatment response. Max: 6 mg bd.
Administration
Should be taken with food: Swallow whole, do not open/crush.
Contraindications
Special Precautions
Patients w/ irregular heartbeat; active stomach ulcer; difficulties passing urine; seizures; asthma or severe resp disease; trembling; low body wt; GI reactions (eg, nausea, vomiting & diarrhoea). May cause dizziness & somnolence, mainly at the start of treatment or when increasing dose. Impaired kidney & liver function. Pregnancy & lactation.
Adverse Reactions
Feeling dizzy, loss of appetite, stomach problems (eg, nausea, vomiting, diarrhoea). Anxiety, sweating, headache, heartburn, wt loss, stomach pain, feeling agitated, feeling tired or weak, generally feeling unwell, trembling or feeling confused, decreased appetite, nightmares. Patients w/ dementia & Parkinson's disease: Trembling, fainting, accidentally falling. Anxiety, feeling restless, slow & fast heartbeat, difficulty in sleeping, too much saliva & dehydration, unusually slow or uncontrollable movements, worsening signs of Parkinson's disease (eg, stiff muscles, difficulty in carrying out movements & muscle weakness). Side effects seen on Rivastigmine transdermal patches which may occur in hard cap: Fever, severe confusion, urinary incontinence.
Drug Interactions
Interference w/ anticholinergics. Risk of stiff limbs & trembling hands w/ metoclopramide. Exaggerated effects of muscle relaxants during anaesth. Risk of bradycardia leading to fainting or loss of consciousness w/ β-blockers (eg, atenolol).
