Duloxetine
Dosage/Direction for Use
Adult 60 mg once daily, taken at the same time each day.
Administration
May be taken with or without food: Swallow whole, do not chew/crush/open cap.
Contraindications
Hypersensitivity. Concomitant use w/ MAOIs (eg, phenelzine, moclobemide) or linezolid; methylene blue in the last 14 days; thioridazine; fluvoxamine; certain antibiotics (eg, ciprofloxacin, enoxacin). Liver impairment or disorder. Severe kidney disease. Childn <18 yr.
Special Precautions
Allergy to any medication. History of liver, kidney, or heart problems, diabetes or seizures. HTN. Low Na levels in the blood. Bleeding disorder or decreased platelet levels. Urination problems. Do not start treatment in patients taking linezolid; wait until 24 hr after last dose of linezolid before starting duloxetine. Angle-closure glaucoma. Changes in behaviour & feelings, thoughts & actions about suicide. Increased risk of breaking a bone. Serotonin syndrome/neuroleptic malignant syndrome (NMS). Alcohol intake or use of street drugs. Fructose intolerance, glucose-galactose malabsorption, sucrose-isomaltase insufficiency. May affect ability to drive & use machines. Pregnancy & lactation.
Adverse Reactions
Constipation, diarrhea, dizziness, dry mouth, erectile dysfunction, feeling tired, headache, insomnia, less appetite, increased sweating, nausea, belly pain, sleepiness, vomiting.
Drug Interactions
MAOIs (eg, phenelzine & moclobemide); linezolid; methylene blue; thioridazine; ciprofloxacin or enoxacin; antidepressants [eg, SSRIs or SNRIs (ie, fluoxetine, venlafaxine, & paroxetine), TCAs (ie, amitriptyline & desipramine), potent CYP1A2 inhibitors (ie, fluvoxamine)]; antipsychotics (eg, olanzapine & risperidone); lithium; oral anticoagulants (eg, warfarin & dabigatran), acetylsalicylic acid, NSAIDs (eg, ibuprofen & naproxen); anti-arrhythmics (eg, flecainide & encainide); medications affecting serotonin (eg, lithium, medications containing tryptophan, St. John's wort, triptans ie, sumatriptan & rizatriptan); opioids (eg, fentanyl, tramadol, tapentadol, mepiridine, methadone & pentazocine); dextromethorphan. Limit alcohol intake while taking duloxetine.
