Human Ig (Per mL: IgG 38 mg, IgA 6 mg, IgM 6 mg)
Indications/Uses
Adjuvant therapy of severe bacterial infections additional to antibiotic therapy. Ig substitution in immunocompromized patients & patients suffering from severe secondary Ab deficiency syndrome.
Dosage/Direction for Use
IV Adult & childn Severe bacterial infection 5 mL (0.25 g)/kg daily on 3 consecutive days at a rate of 0.4 mL/kg/hr. May be administered alternatively w/ the 1st 100 mL at 0.4 mL/kg/hr then 0.2 mL/kg/hr continuously until 15 mL/kg is reached w/in 72 hr. Ig substitution in immunocompromized patients & in severe secondary Ab deficiency syndrome patients 3-5 mL (0.15-0.25 g)/kg. Repeat at wkly intervals if needed. Neonate & infant 5 mL (0.25 g)/kg daily on 3 consecutive days at a rate of 1.7 mL/kg/hr by infusion pump.
Contraindications
Hypersensitivity to human Ig, especially in patients w/ Ab against IgA.
Special Precautions
Recommended infusion rate must be closely followed. Ensure adequate hydration prior to initiation of infusion; monitor urine output & serum creatinine levels; avoid concomitant use of loop diuretics. Risk of hypersensitivity reactions; thromboembolism; acute renal failure; aseptic meningitis syndrome (AMS); haemolytic anaemia. May interfere w/ serological testing (eg, direct Coombs' test). Possibility of transmitting infective agents. Contains glucose & Na. May impair ability to drive & operate machines. Pregnancy & lactation.
Adverse Reactions
Uncommon: Allergic reactions; headache, dizziness; low BP; nausea, vomiting; arthralgia, moderate low back pain; chills, fever.
Drug Interactions
May cause adverse reactions w/ Ca gluconate in infants. Impaired efficacy of live attenuated virus vaccines eg, measles, rubella, mumps & varicella for a period of at least 6 wk & up to 3 mth.
