Indications/Uses
Symptomatic improvement of memory & intellectual impairment of a pathological nature in the absence of a diagnosis of dementia. Improvement in childn w/ learning difficulties associated w/ written word particularly w/ textual understanding. Reduce myoclonus of cortical origin.
Dosage/Direction for Use
Impairment of memory &/or intellectual functions Initially 4.8 g/day in several intakes during the 1st wk of treatment. Maintenance: 2.4 g daily in 2 or 3 divided doses, eventually reducing to 1.2 g daily. Treatment of myoclonus of cortical origin Initially 24 g/24 hr for 3 days. If the response on the 3rd day is weak or absent, continue administration up to the 7th day. If there is still no response or it is inadequate, discontinue treatment on the 7th day. To determine mean active dose: From the day when the dose of 24 g has shown to be active, it should be decreased by 1.2 g every 2 days until myoclonus reappears. Daily dose should be divided into 2 or 3 single doses. Childn 8-13 yr w/ learning difficulties 3.3 g daily.
Administration
May be taken with or without food: May be taken w/ fruit juice or other drinks.
Contraindications
Hypersensitivity to piracetam or other pyrrolidone derivatives. Cerebral haemorrhage, Huntington's chorea, end-stage renal disease (CrCl <20 mL/min).
Special Precautions
Patients w/ severe haemorrhage; patients at risk of bleeding eg, GI ulcer; w/ underlying disorders of haemostasis; history of haemorrhagic CVA; patients undergoing major surgery including dental surgery & patients using anticoagulants or platelet antiaggregant drugs including low-dose aspirin. Avoid abrupt discontinuation of treatment in myoclonic patients. Risk of sickle-cell vaso-occlusive crises in sickle cell patients taking doses <160 mg/kg/day or having irregular drug intake. Slight deterioration of spontaneous movements in patients w/ Huntington's disease taking high doses of Nootropil. Mild laxative effect due to mannitol. Caution in patients w/ phenylketonuria due to aspartame. May cause allergic reactions due to methyl parahydroxybenzoate & propylparahydroxybenzoate. May cause headache, stomach upset & diarrhea due to glycerol. Patients on a controlled Na diet. Patients w/ renal insufficiency. May affect ability to drive or operate machinery. Pregnancy & lactation. Elderly.
Adverse Reactions
Nervousness, hyperkinesia, increased wt.
Drug Interactions
Reports of confusion, irritability & sleep disorder w/ thyroid extr (T3 + T4). Potential interaction w/ acenocoumarol.
