Nitroglycerin
Indications/Uses
Treatment of all forms of episodic cardiac pain (arrestation of angina pectoris attacks). Prophylaxis of angina pectoris. Acute cardiac infarction. Acute myocardial insufficiency, w/ acutely impaired left ventricular function. Catheter-induced coronary spasms during coronary angiography.
Dosage/Direction for Use
1-2 puffs (0.4-0.8 mg), as required. Acute left heart failure or acute cardiac infarction 1-3 puffs (0.4-1.2 mg), as required.
Contraindications
Hypersensitivity. Acute circulatory failure; severe hypotension; cardiogenic shock, unless a sufficiently high left ventricular end-diastolic pressure is assured by intra-aortal counterpulsation or positive inotropic drugs; myocardial diseases concerning reduced cardiac vol; constructive pericarditis; pericardial tamponade; primary pulmonary HTN. Concomitant use w/ certain drugs for erectile dysfunction or pulmonary arterial HTN.
Special Precautions
Careful medical supervision in patients w/ acute cardiac infarction w/ low filling pressures; aortic &/or mitral stenosis; orthostatic disturbances of circulatory regulation; diseases associated w/ increased intracranial pressure. Tolerance development & occurrence of cross tolerance to other nitro compounds. Attenuation of the haemodynamic effects w/in 24 hr. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Headache; decline in BP, orthostatic hypotension.
Drug Interactions
Potentiated antihypertensive effect w/ other vasodilators, antihypertensives, β-blockers, Ca antagonists, neuroleptics or TCAs, alcohol. Enhanced hypotensive effect w/ certain drugs (phosphodiesterase inhibitors) for erectile dysfunction or pulmonary arterial HTN. Patients previously treated w/ organic nitrates (eg, isosorbide dinitrate, isosorbide-5-mononitrate) may require increase in glyceryl trinitrate dose to achieve desired effect. Enhanced hypertensive effect of dihydroergotamine. Weakened effect of heparin.
