Surgical anaesth in epidural block for surgery, including Caesarean section, major nerve block & field block. Acute pain management in continuous epidural infusion or intermittent bolus administration eg, post-op or labour pain & field block.
Adult & childn >12 yr 7.5 mg/mL inj Surgical anaesth: Lumbar epidural in surgery 15-25 mL. Lumbar epidural in Caesarean section 15-20 mL. Thoracic epidural for post-op pain 5-15 mL. Major nerve block (eg, brachial plexus) 10-40 mL. Field block (eg, minor nerve blocks & infiltration) 1-20 mL. 10 mg/mL injSurgical anaesth: Lumbar epidural in surgery 15-20 mL. Childn <12 yr Not recommended for use.
Resuscitative equipment & drugs should be immediately available. Poor general condition due to aging, partial or complete heart condition block, advanced or severe liver disease renal dysfunction. Monitor ECG in patients treated w/ anti-arrhythmics class III. Reports of cardiac arrest. Use w/ caution in patients w/ severe liver disease. Hypotension & bradycardia. Patients on strong CYP1A2 inhibitor therapy. Possibly porphyrinogenic. Chondrolysis in patients receiving post-op intra-articular continuous infusion of local anesth. May impair ability to drive or operate machinery. Pregnancy & lactation.
Additive systemic toxicity w/ other local anaesth or agents related to amide-type local anaesth eg, certain antiarrhythmics (eg, lidocaine & mexiletin). Use w/ caution w/ antiarrhythmic class III eg, amiodarone. Increased plasma conc w/ fluvoxamine, enoxacin & other strong CYP1A2 inhibitors.
