Indications/Uses
Inflammatory disorders eg, primary immune thrombocytopenia when the patient will have surgery in the near future or is at risk of bleeding; Guillain-Barré syndrome; Kawasaki disease. Replacement therapy in cases of primary immunodeficiency syndromes; hypogammaglobulinemia & repeated bacterial infections in patients w/ chronic lymphocytic leukaemia, in whom prophylactic antibiotics have failed, & in plateau phase multiple myeloma patients who have failed to respond to pneumococcal immunisation; hypogammaglobulinaemia in patients after allogeneic haematopoietic stem cell transplantation (HSCT); congenital AIDS w/ recurrent bacterial infections.
Dosage/Direction for Use
IV infusion Replacement therapy in primary immunodeficiency Initially 0.4-0.8 g/kg. Thereafter, 0.2-0.8 g/kg every 3-4 wk. Replacement therapy in secondary immunodeficiency 0.2-0.4 g/kg every 3-4 wk. Congenital AIDS, hypogammaglobulinaemia (<4 g/L) in patients after allogeneic HSCT 0.2-0.4 g/kg every 3-4 weeks. Immunomodulation: Primary immune thrombocytopenia 0.8-1 g/kg on day 1, possibly repeated once w/in 3 days, or 0.4 g/kg/day for 2-5 days. Guillain-Barré syndrome 0.4 g/kg/day for 5 days. Kawasaki disease 1.6-2 g/kg in divided doses over 2-5 days in association w/ acetylsalicylic acid, or 2 g/kg in 1 dose in association w/ acetylsalicylic acid.
Contraindications
Hypersensitivity. IgA-deficient patients, especially those w/ Ab against IgA.
Special Precautions
Recommended infusion rate must be closely followed. Ensure adequate hydration prior to initiation; monitor urine output & serum creatinine levels; avoid concomitant use of loop diuretics. Risk of thrombosis. Potential for hemolysis. Patients suffering from hypo- or agammaglobulinemia; patients who have not received Intratect before or if there has been a long interval; who have been given Intratect recently; who have had reaction to other Abs; overwt patients; diabetics; patients w/ high BP, hypovolaemia, high blood viscosity; bed-ridden or immobilized patients; patients w/ vascular diseases or other risks of thrombotic events. Slow or stop infusion in case of sudden wheeziness, difficulty in breathing, fast pulse, swelling of the eyelids, face, lips, throat or tongue, rash or itching (especially affecting the whole body). Possibility of transmitting infective agents. May affect blood tests. May impair ability to drive & operate machines. Kidney disorder. Pregnancy & lactation. Elderly.
Drug Interactions
Reduced effectiveness of measles, rubella, mumps, chicken pox vaccines.
