Indications/Uses
Dosage/Direction for Use
IV infusion Adult 1st infusion: Initial loading dose: 150 mg/kg in 200 mL soln over 1 hr. 2nd infusion: 50 mg/kg in 500 mL soln over the next 4 hr. 3rd infusion: 100 mg/kg in 1 L over the next 16 hr. Childn Loading dose: 150 mg/kg over 1 hr (150 mg/kg/hr). Given as a 50 mg/mL soln at a rate of 3 mL/kg/hr. 2nd infusion: 50 mg/kg over 4 hr (12.5 mg/kg/hr). Given as a 6.25 mg/mL soln at a rate of 2 mL/kg/hr. 3rd infusion: 100 mg/kg over 16 hr (6.25 mg/kg/hr). Given as a 6.25 mg/mL soln at a rate of 1 mL/kg/hr.
Contraindications
Hypersensitivity to N-acetylcysteine or other chemically correlated substances.
Special Precautions
Risk of anaphylactoid/hypersensitivity reactions particularly w/ initial loading dose. Closely monitor patients w/ bronchial asthma during therapy. Risk of fluid overload in patients w/ body wt <40 kg. May prolong prothrombin time. May interfere w/ colorimetric assay method for salicylate measurement. May interfere w/ urine ketone test. Contains 748 mg Na per vial. Pregnancy & lactation.
Adverse Reactions
Urticaria, rash, pruritus, dyspnoea.
Drug Interactions
Significant hypotension & enhanced temporal artery dilation w/ nitroglycerin. Dissolution of acetylcysteine formulations concomitantly w/ other drugs is not recommended.
