Fludrocortisone acetate
Indications/Uses
Salt-losing adrenogenital syndrome. Partial replacement therapy for primary & secondary adrenocortical insufficiency in Addison's disease.
Dosage/Direction for Use
Addison's disease 0.1 mg daily. May reduce to 0.05 mg daily. Salt-losing adrenogenital syndrome 0.1-0.2 mg daily.
Administration
Should be taken with food.
Special Precautions
May mask some signs of infection. Posterior subcapsular cataract, glaucoma may enhance secondary ocular infections due to fungi or viruses. HTN, oedema, wt gain, fluid & electrolyte imbalance. Lack of antibody response. TB, adrenal insufficiency, hypothyroidism, cirrhosis, herpes simplex, psychic derangements, nonspecific ulcerative colitis, diverticulitis, fresh intestinal anastomoses, active or latent peptic ulcer, renal insufficiency, osteoporosis, acute glomerulonephritis, vaccinia, varicella, exanthema, Cushing's syndrome, antibiotic-resistant infections, DM, CHF, chronic nephritis, thromboembolic tendencies, thrombophlebitis, convulsive disorders, metastatic carcinoma & myasthenia gravis. Pregnancy & lactation. Childn & infant.
Adverse Reactions
Muscle weakness, steroid myopathy, loss of muscle mass, osteoporosis, vertebral compression fractures, aseptic necrosis of femoral & humeral heads, pathologic fracture of long bones & spontaneous fractures. Peptic ulcer, pancreatitis, abdominal distension, ulcerative oesophagitis. Impaired wound healing, thin fragile skin, bruising, petechiae & ecchymoses, facial erythema, increased sweating, subcutaneous fat atrophy, purpura, striae, hyperpigmentation, hirsutism & acneiform eruptions, suppression of reactions to skin tests. Convulsion, papilloedema, vertigo, headache, mental disturbances. Endocrine abnormalities, hyperglycemia, negative nitrogen balance. Necrotising angiitis, thrombophlebitis, aggravation or masking of infections, insomnia, syncopal episodes & anaphylactoid reactions.
Drug Interactions
Reduced efficacy by phenytoin, phenobarb, rifampin. May reduce effects of diuretics, antihypertensives, hypoglycaemics, anticholinesterases.
