Indications/Uses
RA, OA & ankylosing spondylitis. Management of acute pain & dysmenorrhoea.
Dosage/Direction for Use
OA 200 mg daily as a single dose or in 2 divided doses. If necessary, 200 mg bd. RA 100-200 mg bd. Ankylosing spondylitis Initially 200 mg daily as a single dose or in 2 divided doses. If necessary, increase to 400 mg daily after 6 wk. Pain & dysmenorrhoea Initially 400 mg followed by an additional 200 mg, if necessary, on the 1st day; thereafter, 200 mg bd. Elderly Begin at lowest recommended dose. Moderate hepatic impairment Reduce dose by 50%.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to celecoxib or to sulphonamides. Severe heart failure. Not for the treatment of peri-op pain in the setting of CABG surgery. Patients who have experienced asthma, acute rhinitis, nasal polyps, angioneurotic oedema, urticaria or other allergic-type reactions after taking acetylsalicylic acid or NSAIDs including COX-2 inhibitors. Severe CHF (NYHA III-IV). Severe hepatic dysfunction (serum albumin <25 g/L or Child-Pugh score ≥10). Estimated renal CrCl <30 mL/min. Women of childbearing potential unless using an effective method of contraception. Pregnancy & lactation. Childn <18 yr.
Special Precautions
Patients in whom attacks of asthma, urticaria or acute rhinitis have been precipitated by aspirin or nonsteroidal anti-inflammatory agents. Patients w/ CV disease or risk factors for CV disease. GI risk. Not a substitute for acetylsalicylic acid for prophylaxis of CV thromboembolic diseases. Patients w/ history of cardiac failure or HTN. Reports of serious skin reactions including exfoliative dermatitis, Stevens-Johnson syndrome & toxic epidermal necrolysis during post-marketing surveillance. Risk of renal effects. Closely monitor renal function. Elderly.
Adverse Reactions
Abdominal pain, diarrhea, dyspepsia, flatulence, nausea, constipation, gastritis, vomiting, intestinal perforation, colitis w/ bleeding, esophageal perforation, GI bleeding, pancreatitis, ileus; headache, dizziness; insomnia, somnolence, confusion, anxiety; pharyngitis, rhinitis, sinusitis, upper resp tract infection; rash, urticaria, photosensitivity reaction, dermatitis, erythema multiforme, Stevens-Johnson syndrome; aggravated HTN, cerebral ischemia, vasculitis; MI, CHF, palpitation; ecchymosis, epistaxis, thrombocytopenia, anaemia; peripheral edema, HTN, tinnitus, blurred vision, acute renal failure, flushes.
Drug Interactions
May diminish antihypertensive effect of ACE inhibitors. Reduced natriuretic effect of furosemide & thiazides. Increased rate of GI ulceration or other complications w/ aspirin. Increased plasma conc w/ fluconazole. Increased plasma conc of lithium. Caution when co-administered w/ warfarin, 2C9 inhibitors, drugs metabolized by CYP450 2D6.
