Indications/Uses
Mild to moderate Crohn's disease affecting the ileum & ascending colon.
Dosage/Direction for Use
Adult Active Crohn's disease 9 mg (3 cap) daily for 8 wk. Dose in remission: 6 mg (2 cap). Replacement of prednisolone in steroid-dependent patient 6 mg once daily.
Administration
Should be taken with food: Take in the morning. Swallow whole, do not divide/chew. Avoid grapefruit juice.
Contraindications
Special Precautions
Patients w/ infection, HTN, DM, osteoporosis, peptic ulcer, newly created intestinal anastomoses, psychoses, cardiac insufficiency, glaucoma or cataracts, family history of diabetes or glaucoma. Chickenpox & measles may worsen. Can reduce response of HPA-axis to stress. Discontinuation should be gradual. Risk of hypercorticism & adrenal suppression when used chronically in excessive doses. Patients w/ hepatic impairment. Avoid co-administration w/ CYP3A4 inhibitors eg, ketoconazole & grapefruit juice. Risk of adrenocortical suppression in newborn child. Pregnancy. Childn.
Adverse Reactions
Cushing-like symptom; hypokalaemia; nervousness, insomnia & mood swings; reduced visual acuity; palpitations; dyspepsia; urticaria, exanthema; muscle spasms; menstruation disorders.
Drug Interactions
Increased plasma conc w/ ketoconazole, grapefruit juice & other potent CYP3A4 inhibitors. Decreased conc w/ CYP3A4 inducer eg, carbamazepine. Elevated plasma levels & enhanced effects w/ oestrogens & OCs. Possible interaction w/ cholestyramine or antacids.
