Indications/Uses
Dermovate cream/oint Psoriasis (excluding widespread plaque psoriasis), recalcitrant dermatoses, lichen planus & discoid lupus erythematosus; other skin conditions which do not respond satisfactorily to less potent steroids. Dermovate Scalp Application Steroid-responsive dermatoses of scalp eg, psoriasis, recalcitrant dermatoses.
Dosage/Direction for Use
Dermovate cream Appropriate for moist or weeping surfaces. Dermovate oint Appropriate for dry, lichenified or scaly lesions. Adult, elderly & childn >1 yr Apply thinly & gently rub in using only enough to cover the entire affected area once daily or bd until improvement occurs. Then, reduce the frequency of application or change the treatment to a less potent prep. Allow adequate time for absorption after each application before applying an emollient. Max wkly dose should not exceed 50 g/wk. Dermovate Scalp Application Adult, elderly & childn >1 yr Apply a small quantity to the scalp night & morning until improvement is noticeable. It may then be possible to sustain improvement by applying once a day, or less frequently.
Contraindications
Hypersensitivity. Dermatoses in childn <1 yr, including dermatitis. Dermovate cream/oint Untreated cutaneous infections; rosacea; acne vulgaris; pruritus w/o inflammation; perianal & genital pruritus; perioral dermatitis; nappy eruptions in childn <1 yr. Dermovate Scalp Application Scalp infection.
Special Precautions
Caution in patients w/ history of local hypersensitivity to other corticosteroids or to any of the excipients in the prep. W/draw gradually or substitute a less potent corticosteroid in case of manifestations of hypercortisolism (Cushing's syndrome) & reversible hypothalamic-pituitary-adrenal (HPA) axis suppression. Avoid long-term continuous topical corticosteroid therapy in infants & childn <12 yr, courses should be limited if possible to 5 days & reviewed wkly; occlusion should not be used. Infection risk w/ occlusion. Use w/ caution in patients w/ psoriasis. Appropriate antimicrobial therapy should be used whenever treating inflammatory lesions which have become infected. Patients w/ renal/hepatic impairment (in case of systemic absorption). Pregnancy & lactation. Elderly. Dermovate cream/oint Higher occurrence of local hypersensitivity reactions & increased risk of local infection when used to treat dermatitis around chronic leg ulcers. If applied to the face, courses should be limited to 5 days if possible & occlusion should not be used. If applied to the eyelids, care is needed to ensure that the prep does not enter the eye, as cataract & glaucoma might result from repeated exposure. If Dermovate does enter the eye, the affected eye should be bathed in copious amounts of water. Contains propylene glycol which may cause skin irritation. Dermovate cream Contains cetostearyl alcohol & chlorocresol. Dermovate Scalp Application Avoid contact w/ eyes. Avoid smoking whilst applying to the scalp; fire, flame & heat including use of hair dryer after application. Visual disturbance.
Adverse Reactions
Drug Interactions
Increased systemic exposure w/ CYP3A4 inhibitors (eg, ritonavir, itraconazole).
