Methylphenidate HCl
Indications/Uses
Treatment of ADHD in patients ≥6 yr.
Dosage/Direction for Use
Methylphenidate-naive patients or patients on stimulants other than methylphenidate Adult 18-65 yr 18 mg or 36 mg. Childn & adolescent 12-17 yr 18 mg. Max: Adult & adolescent >13 yr 72 mg daily. Childn 6-12 yr 54 mg daily. Patient currently using methylphenidate 5, 10, 15 & 20 mg bd or tds is converted to 18, 36, 54 & 72 mg every morning, respectively. Dosage may be increased in 18 mg increments at wkly intervals. To be taken once daily in the morning.
Administration
May be taken with or without food: Swallow whole, do not divide/chew/crush.
Contraindications
Hypersensitivity. Patients w/ marked anxiety, tension & agitation, glaucoma, family history or diagnosis of Tourette's syndrome. Concomitant or w/in 14 days of MAOI therapy.
Special Precautions
Patients w/ structural cardiac abnormalities. Personal or family history of tics. Interrupt treatment if patients are not growing or gaining wt. Do not administer w/ pre-existing severe GI narrowing or dysphagia. Not for treatment of severe depression &/or for treatment/prevention of normal fatigue states. Discontinue if w/ psychotic or manic symptoms. Patients w/ underlying medical conditions which might be compromised by BP or heart rate increases. History of drug dependence or alcoholism. Seizures or prior EEG abnormalities in absence of seizures. Psychotic patients. Periodic haematologic monitoring is advised during prolonged therapy. Visual disturbances. May affect ability to drive or operate machinery. Pregnancy & lactation. Elderly >65 yr. Childn <6 yr.
Adverse Reactions
Adult Decreased appetite, anorexia, insomnia, anxiety, depressed mood, restlessness, agitation, headache, dizziness, tremor, tachycardia, palpitations, dry mouth, nausea, hyperhidrosis, irritability, decreased wt. Paed Upper abdominal pain, insomnia, dizziness, pyrexia.
Drug Interactions
MAOIs, vasopressor agents, coumarin anticoagulants, anticonvulsants eg, phenobarb, phenytoin, primidone, certain antidepressants eg, TCAs, SSRIs & clonidine.
