Indications/Uses
Inj: Treatment of some serious types of infections caused by certain bacteria, when other antibiotics are not suitable. Inhalation: For chronic chest infections caused by Pseudomonas aeruginosa in patients w/ cystic fibrosis.
Dosage/Direction for Use
Infusion or inj Adult Usual daily dose: 9 MU, divided into 2-3 doses, as an infusion over 30-60 min. Max: 12 MU. Childn up to 40 kg 75,000-150,000 u/kg, divided into 3 doses. Higher doses have occasionally been given in cystic fibrosis. Renal impairment Lower doses are usually given. Inhalation Adult, adolescent, childn ≥2 yr 1-2 MU bd-tds. Max: 6 MU/day. Childn <2 yr 0.5-1 MU bd. Max: 2 MU/day.
Contraindications
Hypersensitivity to colistimethate Na, colistin or to other polymyxins.
Special Precautions
Some patients may experience a feeling of tightness in the chest when inhaling Colomycin. Myasthenia gravis, porphyria, asthma. Do not drive or operate machinery when side effects eg, dizziness, confusion or problems w/ vision occur. Renal impairment. Pregnancy & lactation. Premature & newborn babies.
Adverse Reactions
Allergic reactions. Inj: Difficulty in breathing due to chest muscle paralysis; numbness or tingling, dizziness or loss of balance, rapid changes in BP or blood flow, slurred speech, problems w/ vision, confusion & mental problems; inj site reaction eg, irritation; kidney problems. Inhalation: Coughing, feeling of tightness in the chest, sore mouth or throat, thrush infections of the mouth or throat.
Drug Interactions
Increased risk of nephrotoxicity & neurotoxicity w/ aminoglycosides (eg, gentamicin, tobramycin, amikacin, netilmicin). Increased risk of nephrotoxicity w/ cephalosporins. Increased effects of muscle relaxants. Increased risk of muscle weakness & breathing difficulties w/ macrolides or fluoroquinolones in patients w/ myasthenia gravis.
