Clopidogrel
Indications/Uses
Prevention of atherothrombotic events in MI, ischemic stroke or established peripheral arterial disease. Acute coronary syndrome: Non-ST segment elevation acute coronary syndrome (unstable angina or non-Q wave MI), including patients undergoing stent placement following percutaneous coronary intervention, in combination w/ aspirin; ST segment elevation acute MI, in combination w/ aspirin in patients eligible for thrombolytic therapy.
Dosage/Direction for Use
Adult & elderly 75 mg once daily. Non-ST segment elevation acute coronary syndrome (unstable angina or non-Q-wave MI) A single 300-mg loading dose & then continued at 75 mg once daily w/ ASA (75-325 mg daily). Since higher doses of ASA were associated w/ higher bleeding risk it is recommended that the dose of ASA should not be higher than 100 mg. ST segment elevation acute MI A single daily dose of 75 mg initiated w/ a 300-mg loading dose in combination w/ ASA, & w/ or w/o thrombolytics.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to clopidogrel, soya, peanut. Severe liver impairment; active pathological bleeding eg, peptic ulcer or intracranial haemorrhage. Concomitant use w/ repaglinide.
Special Precautions
Patient who may be at risk of increased bleeding from trauma, surgery or other pathological conditions. Discontinue 7 days prior to surgery if patient is to undergo elective surgery. Lesions w/ propensity to bleed (particulary GI & intraocular). Thrombotic thrombocytopenic purpura. Recent ischaemic stroke (7 days after acute ischaemic stroke). Concomitant use w/ CYP2C19 inhibitors. Renal impairment, moderate hepatic impairment. Pregnancy & lactation.
Adverse Reactions
Haematoma; epistaxis; GI haemorrhage, diarrhoea, abdominal pain, dyspepsia; bruising; bleeding at puncture site.
Drug Interactions
Increased intensity of bleeding w/ oral anticoagulants. Caution when in concomitant w/ aspirin, heparin, thrombolytics, glycoprotein IIb/IIIa inhibitors, NSAIDs including COX-2 inhibitors. Use of CYP2C19 inhibitors (eg, omeprazole & esomeprazole, fluvoxamine, fluoxetine, moclobemide, voriconazole, fluconazole, ticlopidine, ciprofloxacin, cimetidine, carbamazepine, oxcarbazepine & chloramphenicol) is discouraged. Decrease exposure of active metabolite of clopidogrel w/ proton pump inhibitors (eg, omeprazole, esomeprazole).
