Indications/Uses
Symptomatic relief in the treatment of OA, RA & ankylosing spondylitis in adults.
Dosage/Direction for Use
OA One 200 mg cap once a day, increase to max 400 mg if needed. RA Max 400 mg, if needed. Ankylosing spondylitis One 200 mg cap once a day, increase to max 400 mg if needed.
Administration
May be taken with or without food: Dose for OA/RA may be given w/ or w/o meals.
Contraindications
Hypersensitivity to celecoxib or to sulphonamides. Active peptic ulceration or GI bleeding. Patients who have experienced asthma, acute rhinitis, nasal polyps, angioneurotic oedema, urticaria or other allergic type reactions after taking acetylsalicylic acid or other NSAIDs including COX-2 inhibitors. Inflammatory bowel disease. CHF NYHA II-IV. Established ischemic heart disease, peripheral arterial disease &/or cerebrovascular disease. Patient w/ severe heart failure. Treatment of peri-op pain in the setting of CABG surgery. Patients w/ estimated CrCl <30 mL/min. Severe hepatic dysfunction. Women of childbearing potential unless using an effective method of contraception. Pregnancy & lactation.
Special Precautions
History of GI ulcer or bleeding; heart, liver or kidneys dysfunction; smoking, DM, raised BP or cholesterol; fluid retention; dehydration eg, due to sickness, diarrhea or use of diuretics; serious allergic reaction or serious skin reaction to any medicines; infection or illness due to an infection. May cause an increased risk of serious CV thrombotic events, MI, & stroke; serious GI adverse events including bleeding, ulceration, & perforation of the stomach or intestines. Risk of renal papillary necrosis & other renal injury in long-term NSAID administration; renal toxicity in patients in whom renal prostaglandins have a compensatory role in the maintenance of renal perfusion. Not recommended in patients w/ advanced renal disease. Concomitant use w/ anticoagulants; acetylsalicylic acid; corticosteroids. Avoid concomitant use w/ other non-acetylsalicylic NSAID eg, ibuprofen or diclofenac. Alcohol consumption. Elderly >65 yr. Not for use in childn.
Adverse Reactions
Allergic reaction eg, skin rash, swelling of the face, wheezing or difficulty in breathing; heart problems eg, chest pain; severe stomach pain or any sign of GI bleeding eg, passing black or bloodstained stools or vomiting blood; skin reactions eg, rash, blistering or peeling of the skin; liver failure.
Drug Interactions
Dextromethorphan; ACE inhibitors, AIIA, β-blockers & diuretics; fluconazole & rifampicin; warfarin or other warfarin-like medicines; lithium; other medicines to treat depression, sleep disorders, high BP or an irregular heartbeat; neuroleptics; methotrexate; carbamazepine; barbiturates; ciclosporin & tacrolimus.
