Hyoscine butylbromide
Indications/Uses
Tab Relief of GIT & GUT cramps. Symptomatic relief of irritable bowel syndrome (IBS). Inj Relief of acute GUT or GI spasm (eg, renal or biliary colic), or to produce smooth muscle relaxation prior to radiological procedures eg, pyelography or other diagnostic procedures where spasm may be a problem (eg, gastroduodenal endoscopy).
Dosage/Direction for Use
Tab Adult & childn >12 yr 2 tab qds. IBS Initially 1 tab tds, may be increased if necessary. Childn 6-12 yr 1 tab tds. Inj Adult 1 amp (20 mg) IM, SC, or IV, at a slow inj rate of 1 mL/min. Max: 100 mg/day (5 amp).
Administration
May be taken with or without food: Tab: Swallow whole, do not break/crush/chew.
Contraindications
Hypersensitivity to hyoscine butylbromide or atropinics. Patients w/ myasthenia gravis, untreated narrow angle glaucoma, stenotic lesions or mechanical stenosis of the GIT, paralytic or obstructive ileus, & megacolon. Tab Women of childbearing potential, pregnancy & lactation. Inj Patients w/ prostatic hypertrophy w/ urinary retention, tachycardia, angina, cardiac failure. Do not give by IM in patients on anticoagulants.
Special Precautions
Tab Patients w/ very fast heart rate or other heart problems; thyroid gland disorders (eg, overactive thyroid gland); difficult or painful urination (eg, men w/ prostate problems); constipation; fever. May affect ability to drive or operate machinery. Contains sucrose. Not recommended for childn <6 yr. Inj Risk of anaphylaxis, including episodes of shock; tachycardia & hypotension. Patients intolerant of other belladonna alkaloid or derivative; patients w/ cardiac conditions; unstable CV status in an acute hemorrhage situation; patients inclined to tachyarrhythmia; patients w/ reflux esophagitis or GIT obstruction disease. May aggravate hiatal hernia associated w/ reflux esophagitis, myasthenia gravis or pyloric obstruction. Large anticholinergic doses may suppress intestinal motility in patients w/ ulcerative colitis; may precipitate or aggravate toxic megacolon. May precipitate or aggravate urinary retention in patients w/ non-obstructive prostatic hypertrophy, urinary retention (or predisposition to) or obstructive uropathy eg, bladder neck obstruction due to prostatic hypertrophy. May result in increased IOP. Transient disturbances of accommodation which recede spontaneously. Exercise caution while driving or operating machinery. Preferable to avoid during pregnancy & lactation. Not recommended in childn. Geriatric patients.
Adverse Reactions
Tab Severe hypersensitivity (eg, difficulty breathing, feeling faint or dizzy); allergic reactions eg, skin reactions (nettle rash, itching, rash, skin redness); painful red eye w/ loss of vision. Dry mouth, abnormal or reduced sweating, increased heart rate, urinary retention. Inj Tachycardia; visual accommodation disorders, mydriasis, increased IOP; dry mouth; urinary retention; hypohidrosis, heat sensation/transpiration.
Drug Interactions
Tab Potential interaction w/ tetracyclic antidepressants or TCAs eg, doxepin; antihistamines; quinidine, disopyramide; antipsychotics eg, haloperidol or fluphenazine; tiotropium, ipratropium or atropine-like medicines; amantadine; metoclopramide. Inj Can potentiate anticholinergic effect of TCAs & tetracyclic antidepressants; antipsychotics; atropine-like compd; antihistamines; quinidine; disopyramide; amantadine; MAOIs; anticholinergics. May increase severity of KCl-induced GI lesions. Concomitant use w/ dopamine antagonists (eg, metoclopramide) may result in diminution of the effects of both drugs on GIT. May enhance tachycardic effects of β-adrenergic agents. Potential reduced absorption w/ antacids or adsorbent antidiarrheals.
