Bleomycin HCl
Indications/Uses
Skin cancer, head & neck cancer, lung cancer (particularly, primary or metastatic squamous cell carcinoma), esophageal carcinoma, malignant lymphoma, uterine cervical cancer, neuroglioma, thyroid cancer.
Dosage/Direction for Use
IV 15-30 mg in approx 5-20 mL solvent. When severe fever is noted, ≤5 mg/dose. IM & SC 15-30 mg in approx 5 mL solvent. When administered SC around the affected area, ≤1 mg/mL. Intra-arterial 5-15 mg. Administer twice wkly, may be adjusted to once daily or once wkly. Total administered dose to achieve tumor clearance: ≤300 mg.
Contraindications
Hypersensitivity to bleomycin or a similar drug eg, peplomycin. Patients w/ serious pulmonary function impairment or w/ chest X-ray findings suggesting diffuse fibrotic changes or any other remarkable changes; serious renal function disorder; serious heart disease; receiving radiation therapy to the chest & surrounding area.
Special Precautions
History or complication of lung disorder; renal, heart, liver disorders; prior radiation therapy to the chest; chicken pox. Monitor for onset of crepitations (rales). Observe patients for clinical symptoms eg, fever, cough, exertional dyspnea, etc. Examine alveolar-arterial oxygen tension difference (A-aDO2), arterial oxygen tension (PaO2), carbon monoxide diffusing capacity (DLCO), etc during treatment & until approx 2 mth after completion of administration. Discontinue in case of an increase in A-aDO2 or decrease in PaO2 during 2 consecutive wk. Long-term administration. Concurrent treatment w/ peplomycin or other bleomycin products. Monitor for onset or exacerbation of infection & any bleeding tendency. MI, cerebral infarction due to concomitant use w/ other anticancer drugs. Fibrosarcoma & renal carcinoma in SC administration. Pregnancy & lactation. Childn. Elderly ≥60 yr. Patients of reproductive age.
Adverse Reactions
Interstitial pneumonia & pulmonary fibrosis, dermal sclerosis & pigmentation, fever & chills, alopecia, anorexia & decreased wt, general malaise, nausea & vomiting, stomatitis, nail changes. Rash, urticaria, erythroderma associated w/ fever; angular stomatitis; headache.
Drug Interactions
Possible occurrence of serious pulmonary manifestation eg, interstitial pneumonia & pulmonary fibrosis w/ irradiation to the chest & surrounding area; other antineoplastic agents, irradiation in general. Possible inflammation of mucosa w/ irradiation to the area of head & neck.
